**“Health For Two is a free program for women who need extra support to have a healthy pregnancy. Health for Two is a prenatal program that provides support, information, and resources throughout pregnancy and until 2 months post-partum to women who require extra support due to health and/or social issues.”**

The Health for Two Support Worker works primarily offsite in the community providing services to pregnant women and support to community agencies. Health for Two is delivered in all Public Health Centers in the Edmonton Zone and in over 45 different community agencies and is funded through the Public Health Agency of Canada and works accordingly with AHS staff and guidelines

**Position Responsibilities**
- Provide case management services to clients on the Health for Two Program, working closely with partner agencies, Public Health Centre nurses, and/or Hf2 Network Nurses
- Build rapport with families that are referred to the program
- Assess client’s needs in order to provide assistance, support, and advocacy.
- Build and maintain positive relationships with agency and Alberta Health Services staff and work collaboratively on referrals, case management and consultation activities
- Provide emotional support and social support to clients, including cultural supports
- Maintain up-to-date knowledge on various community and government programs
- Maintain accurate comprehensive documentation regarding all interactions with clients in accordance with agency standards
- Attend relevant Hf2 meetings including Network Meetings, Network Team meetings, Hf2 Team Meetings, and Support Worker/Coordinator meetings
- Work collaboratively with Network Coordinator to meet the needs of the agency partners, health centres, and the network itself.
- BSCS is more than a workplace. It is a community where everyone helps and contributes to the whole, and there is an expectation that all employees will take part in events (during the work week and occasionally during weekends and evenings). This includes fundraising campaigns, running community meals, medicine picking etc.

**Qualifications**
- Post Secondary Degree in a relevant field of study (Social Work, Social Sciences) and/or relevant work experience
- At least 2 years’ experience delivering community-based services
- Experience networking and developing relationships with a range of partners and stakeholders
- Excellent knowledge and understanding of social determinants of health including poverty, mental health and addictions, trauma etc.
- Police Check with the Vulnerable Sector and Child and Youth Intervention Check completed within the past six months
- Valid First Aid certificate or enrolled in a future course
- Valid Class 5 Non GDL license and access to a reliable vehicle
