Shared Health leads the planning and coordinates the integration of patient-centred clinical and preventive health services across Manitoba. The organization also delivers some province-wide health services and supports centralized administrative and business functions for Manitoba health organizations.

**Position Summary**:
The Palliative Care Coordinator reports to the Manager - Palliative Care, and works in collaboration with health care providers and community supports in the delivery of palliative care services within the Southern Health-Santé Sud. The Palliative Care Coordinator provides professional nursing services to culturally diverse clients, in a variety of settings, inclusive of home, long term, transitional, and acute care settings and acts a resource for clients, families and staff. The Palliative Care Coordinator has responsibility for the delivery of education to Nurses, Health Care Aides and volunteers in acute, transitional, long-term and community regarding Hospice Palliative Care theory, and specialized skills. The incumbent will act in accordance with Southern Health-Santé Sud Board Ends, core values, policies and procedures and will adhere to the Norms of Practice developed by the Canadian Hospice Palliative Care Association, the College of Registered Nurses of Manitoba Standards of Practice, and the Canadian Nurses Association Code of Ethics. The incumbent exercises the appropriate level of initiative and independent judgment in determining work priorities, work methods to be employed and action to be taken on unusual matters. The possition functions in a manner that is consistent with the mission, vision and values; and the policies of Southern Health-Santé Sud.

**Qualifications**:

- Graduate of a Diploma or Baccalaureate Nursing program.
- Canadian Nursing Association palliative care certification preferred.
- Additional formal education in palliative care.
- Other combinations of education and experience may be considered.
- Proficiency with MS Office, Outlook, and Excel.
- Minimum three (3) years’ experience working with palliative individuals in a community-based or facility setting.
- Integrity in dealing with confidential matters.
- Good interpersonal skills and the ability to work in a team setting at all levels of the organization.
- Strong organization, decision making, flexibility, analytical and problem-solving skills.
- Excellent written and verbal communication skills.
- Demonstrate ability to work and make decisions both interdependently and independently when required.
- Given the cultural diversity of our region, the ability to respect and promote a culturally diverse population is required
- Proficiency of both official languages is essential for target and designated bilingual positions
- Demonstrated ability to meet the physical and mental demands of the job
- Demonstrated ability to respect confidentiality including paper, electronic formats and other mediums
- Good work and attendance record

**Conditions of Employment**:

- Completes and maintains a satisfactory Criminal Record Check, Vulnerable Sector Search, Adult Abuse Registry Check and Child Abuse Registry Check, as appropriate.
- All Health Care workers are required to be immunized as a condition of employment in accordance with Southern Health-Santé Sud policy.
- Requires a valid Class 5 driver’s license, an all-purpose insured vehicle and liability insurance of at least $1,000,000.00.
