**Care Coordinator required for seniors' living facility**

Green Acres Foundation is looking for someone with at least 3 years' experience in nursing who has a flexible, willing, and positive attitude. The right applicant will have successfully completed an approved nursing program and have proof of an active registration with CARNA or CLPNA. A current satisfactory criminal record check is required, and related nursing experience in geriatric care is an asset.

**About the position**:

- This is a Temporary Full-Time position.
- Must be available to work weekdays, weekends, and holidays.
- Shifts to cover in this position are: 6am to 2pm, 2pm to 10pm, and/or 10pm to 6am.

**Basic Responsibilities of the position include**:

- Oversee nursing and personal care services in a lodge setting.
- Provide supervision and leadership to care staff in a manner that motivates, guides, and directs employees, promotes staff participation, teamwork, and positive relations.
- Participate in the development and coordination of a comprehensive resident centered plan of care.
- Work collaboratively with Home Care and other members of the inter-disciplinary team.
- Assist with the admission, discharge, and transfer processes for all residents receiving care services in accordance with care plans and as requested by Site Management.
- Maintain effective relationships with the residents, families, and AHS.
- Perform in scope nursing duties as needed and as directed by Site Management.
- Perform other duties as assigned.
