Social Service Worker Lead

St Catharines, Canada St. Joseph's Villa - Behavioural Support Ontario (BSO) Full time

The HNHB LHIN BSO Behavioural Lead will use a team approach to provide leadership, mentorship and support to the LTCH team in the development of a comprehensive, multidisciplinary plan of care.

The Behavioural Lead will oversee the coaching, mentoring and modeling strategies that are to be implemented as well as other relevant interventions to manage responsive behaviours. By ensuring a commitment to client centered resident care, the lead will foster a culture that is positive, creative, respectful, flexible, and enthusiastic.

**Duties and Responsibilities**:
1. Works collaboratively with care partners and consultants serving long term care homes. This includes HNHB Placement Services, Alzheimer Society PRCs (Psychogeriatric Resource Consultants, Specialized Geriatric Services, Community BSO services, LTCH BSO Mobile, BSO Transitional Leads and discharge planners and other health service provided as required.

2. Assist residents in dealing with personal and social problems by delivering counseling, connection with community services and social supports as well as counseling for daily/ social skills.

3. Completes required/ identified assessments and the development of strategies/recommendations for the targeted risk population including pre-admission, admission, transitional and episodic cases/ referrals from opening the case within the client’s home to case completion or close within the assigned LTCH.

4. Participates in Continuous Quality Improvement practices as identified by the BSO Program Strategic Lead, its Director or Oversight Committee through data collection, analysis, improvement initiatives and PDSA cycles as directed/ requested.

5. Supports the resident their caregivers by providing counselling, group or individual according to the College of Social Work and Social Service Workers scope of practice.

6. Provides recommendations on bio-psychosocial and non-pharmacological interventions to support resident goals related to psychosocial issues.

7. Responsible for initial PIECES and other relevant assessments on all new referrals within assigned hours.

8. Ensures care strategies progress and are regularly evaluated and adapted in consultation with the LTCH care team.

9. Utilizes and completes approved assessment tools as per established BSO practice and scope of practice.

10. Provides mentorship and support that is person-centred and based on best practices.

11. Identifies themes and opportunities for educational in-services in collaboration with the LTCHs, other service providers, outreach services/teams and experts in the field.

12. Utilizes therapeutic communication by establishing a therapeutic relationship with clients, families, co-workers and others for the purpose of meeting client needs.

13. Collaborates with LTCH staff/leadership, families, and service providers in a professional, courteous manner and keeps the BSO Mobile Team Lead informed of any changes or concerns identified.

14. Provides support to the care team and their CSW by also trialling, modelling and coaching strategies to mitigate escalating responsive behaviours to LTCH staff where appropriate and in support of the reason for referral.

15. Participates in ongoing education and training related to best practices and responsive behaviours to the target population.

16. Attends Family Council meetings in the assigned LTCH as invited by Council in collaboration with the LTCH Senior Management to explain role and provide education.

17. Participates in responsive behaviour meetings within the assigned LTCH, attends all BSO regional staff meetings, education and/or training as required by the host employer St. Joseph’s Villa.

18. As a team, support successful resident transitions by facilitating implementation of care plan recommendations identified by the BSO Transitional or Clinical Leader in shared clients/ residents.

19. Identify opportunities to build capacity of LTCH staff through mentorship and coaching.

20. Respond in emergency situations (e.g., fire alarms, missing resident, bomb threat & evacuation).

21. Carry out all duties with appropriate attention for resident/client safety.

22. Responds in Emergency situations such as bomb threat, fire, Code Yellow, Code White as per established procedure in the LTC home and only at the request of the LTC home as a guest.

23. Responsible for ensuring privacy and confidentiality of personal health information in keeping with wishes or consent and capacity of the resident.

24. Maintains public health information according to college practices and applicable laws

25. Complete other duties as assigned by the BSO Mobile RN Manager.

**Supervisory Duties**:
26. Supervises the practice, accountability, performance and conduct of the BSO Mobile CSW (Care Support Worker) assigned to the targeted Behavioural Lead home(s).

27. Participates in the interview and selection process of BSO Care Support Workers assigned to the targeted homes.

28. Completes the probation
