District Coordinator

The District Program Coordinator (PC) empowers individuals with physical and intellectual disabilities as well as mental health challenges by enabling them to live their best lives in Nova Scotian communities. This includes
- though is not limited to -active community engagement, support opportunities for social inclusion, life skill development and assistance with activities of daily living (ADL). The District PC adheres to Active Support and least restrictive practices while establishing and maintaining meaningful relationships with the individuals they support.
Reporting to the Jr/Manager of Participant Services, the District PC is a full-time coordinator position required to operate with a high level of independence in providing direct support to MCL’s participant population and providing leadership to MCL’s direct support staff. The District PC is also responsible for the daily planning, organization, and supervision of MCLs programs and services.
Regular PC working hours are 8:30am-4:30pm, Monday-Friday. However, the hours of work for the District PC position are flexible and can be unpredictable to respond to participant and programming needs.
**Position Overview**:
- **Business Development **_
- Responsible for outsourcing growth opportunities for the company.
- Responsible for networking with key community stakeholders such as the Disability Support Program (DSP) representatives.
- Responsible for establishing regular contact with community partners for improved service delivery.
- **Participant Planning and Support**_
- Responsible for maintaining strict confidentiality on all matters pertaining to the participants served.
- Responsible for working collaboratively with the participant and their care team(s) to ensure the Commitment to Care Plan is being upheld.
- Responsible for working collaboratively with the CRW (ILS) and participant treatment team(s).
- The PC is responsible for providing direct participant support and documentation as required.
- Provides supervision to the CRW (ILS) to ensure that participants are attending all medical, mental health and therapeutic appointments and that treatment recommendations are implemented.
- Ensures proper documentation and reporting is being completed by the CRW (ILS).
- Responsible for collaborating with the CRW (ILS) to complete Individual Program Plans (IPP) and quarterly reports, proper documentation for medical and mental health records and information from relevant others.
- Responsible for fostering positive working relationships with community partners.
- Ensures participant Special Needs Requests are identified by the CRW (ILS) and submitted to the Care Coordinator in a timely manner.
- Identifies and documents any concerns reported by participants and/or family members, informs the Manager of Participant Services and ensures that all action plans and necessary follow up are completed and documented in a timely manner.
- Determines the level of intervention and/or support required in the event of a crisis situation or incident and take measures in an attempt to ensure the appropriate level of support is provided to the participant. The PC will develop and implement emergency response protocols for the participants as is appropriate and necessary.
- Participates in the referral and intake process, completing Commitment to Care Plans (CCP) and discharge planning.
- Performs other related duties as assigned by the Manager of Participant Services.
- **Operations**_
- Responsible for ensuring direct support hours are being fulfilled as approved by the DSP Care Coordinator.
- Provides leadership to the CRW (ILS) and management in the planning and implementation of extra staffing hours. Ensures the budgeted extra staffing hours are utilized effectively.
- The PC will work collaboratively with the CRW (ILS) to ensure that participants' environments are in accordance with MCL’s program standards as well as health, fire and safety regulations. In addition, the PC will report the need for property repair and /or attention to the Property Manager.
- Responsible for ensuring participants remain in good standing with rental increases and tenant insurance policies.
- Participates in MCL’s Emergency on Call (EOC) schedule.
- Participates in MCL’s Scheduling on Call (SOC) schedule as needed.
- Performs other related duties as assigned by the Manager of Participant Services.
- **Human Resource Management **_
- Ensures orientation of new direct support employees.
- Provides leadership, direction, motivation and guidance to the CRW (ILS)
- Administration and interpretation of NSGEU collective agreement.
- Conducts CRW (ILS) probationary and annual reviews.
- Investigates, documents and follows up on any matters regarding staff performance and discipline, including terminations.
- Schedules and chairs regular staff meetings.
- Identifies staff training needs and assists the Manager of Participant Services with the coordination of staff training and
