**Your Opportunity**:
The Social Worker II (SWII) position is a generalist role that will collaborate with Contracted Service Providers (CSP) to help navigate situations where Facility Living (Long Term Care) residents require enhanced system navigation in areas such as: decision making, legal matters, financial challenges, family dynamics etc. This position provides the opportunity to work independently in a dynamic, self
- directed environment. You will work with an interdisciplinary team in the community and zone. Travel to sites within the AHS Edmonton Zone required. Key Responsibilities: Timely and thorough assessments to inform on best course of action. Facilitates/informs on relevant services; supports coordination of resources. Maintains records as per policies and regulatory body requirements. Sound judgment and decision-making skills based on the SW scope of practice and individual career experience. Effective interpersonal skills: work with multiple stakeholders through system pressures where funding and goals do not align. Professional written, verbal, and non-verbal communication skills based on audience, individual role, and situation. Resident-family centered approach to care. Creative problem-solving skills to support CSP SWs. Representative on committees and working groups. Ability to adapt to change. Build key working networks with internal and external stakeholders.
**Description**:
As a Social Worker II, you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families. Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.

**Required Qualifications**:
Completion of bachelor's degree in Social Work. Active or eligible for registration with the Alberta College of Social Workers (ACSW).
**Additional Required Qualifications**:
Minimum, five years of experience working with the geriatric and/or geriatric mental health population. Working knowledge of relevant and important stakeholders. Computer literacy.
**Preferred Qualifications**:
Experience in Facility Living.
