**Bereavement Coordinator**
**12 Month Contract**

**Overview of position**:

- Coordinate Bereavement Services
- Organize and implement program development based on the needs of the bereavement clients and available resources
- Engage and coordinate volunteer resources to support the Bereavement program

**Duties and Responsibilities**:

- Provide leadership for Bereavement Programs including in intake, one-one support, pre-group orientation and psychoeducational groups.
- Review referrals and redirect inappropriate referrals to other community resources.
- Conducts assessments to determine most appropriate type of bereavement support for the individual and assist clients in developing care plans.
- Completes required documentation of all assessments, telephone contacts and follow-up visit or contacts.
- Provide leadership, training, and support to Bereavement Volunteers including volunteer debrief and support meetings.
- Provide timely and accurate statistical reporting for individual clients and bereavement groups.
- Provide support for strategic planning, accreditation, quality assurance, fund-raising and other activities of Hospice Simcoe
- Collaborate with external community partners to strengthen bereavement services offered to clients experiencing various types of loss
- Models a work culture that promotes self-care, collaboration, diversity and positive regard for patients/families, colleagues, and volunteers.

**Skills and qualifications**
- University Degree in Social Work (BSW), MSW an asset
- Current registration with Ontario College of Social Workers
- Experience in grief and bereavement in community settings
- Excellent interpersonal, oral and written communication skills
- Experience working with a culturally diverse audience
- Ability to work as part of a team in a service-oriented environment
- Ability to prioritize and oversee a high volume of work in a timely and efficient manner
- Ability to use Microsoft Office, databases and the Internet

**Salary**: $33.00-$37.00 per hour
