**Reporting to the **Manager of Integrated Client Services, **the incumbent will be responsible for the following functions**:

- Engaging individuals at their initial point of contact with the Criminal Justice system.
- Completing the referral and assessment process to determine individual needs and services.
- Acting as a resource to CMHA staff and other service providers in the Municipality of Chatham Kent.
- Liaising with correctional facilities to provide assessments in preparation for discharge.
- Assessing and determining eligibility for Court Diversion.
- Serve as access and coordination points for the service and criminal justice systems.
- Develop linkages and/or protocols with service providers to ensure continuity of care and reduce risk of future recidivism.
- Provide assertive outreach to treatment-resistant individuals though the period of their court matters.

**Qualifications**:

- Post-secondary degree in Social Work from an accredited university with current registration with the O.C.S.W.S.S.W; a Registered Nurse or Registered Practical Nurse with current registration with the C.NO. (Preference to those with Canadian Certification in Psychiatric/Mental Health Nursing); Bachelor’s Degree with a Social Service Worker Diploma and a current registration with the O.C.S.W.S.S.W; or a post-secondary degree with a relevant professional registration.
- Experience and a detailed knowledge of serious mental illness, usually gained by several years of working with this population
- Valid Ontario Driver’s Licence and access to a vehicle are required.

**Hours of Work**: 35 hours/week. Flexible hours are required to meet the position requirements.

**Salary**: Commensurate with experience and education within the range: $53,393- $75,995

Inclusive of HOOPP.
