**ACCOUNTABILITY OBJECTIVE**

Are you passionate about making a difference in the lives of seniors? Our Health and Wellness Attendants at The Lodge at Valley Ridge are critical to our community by supporting our residents to do the things that matter most to them while providing a comfortable, safe and nurturing environment. Under the direction of the Health and Wellness Manager, the Health and Wellness Attendants position is focused on enriching residents lives by ensuring they can make their own choices and lead full lives as independently as they can. Delivers care to residents based on individualized care plan, policies and procedures, and established standards of care.

**BACKGROUND/QUALIFICATIONS**

Has obtained a certificate from an approved Health Care Aide program. Has completed a minimum of grade 12. Preference given to individuals with previous experience in working with seniors. Written and verbal fluency in English is required. A resident-oriented individual, whose visions and values align with those of Verve.

**KEY RESPONSIBILITIES**- Participates in continuous quality improvement standards.
- Collaborates with the care team to provide feedback on care plans for the residents based on their physical, emotional, social and safety needs.
- Provides compassionate and respectful care to residents including cognitive, social and emotional support, according to their care plan and in a manner that promotes comfort, safety, and well-being.
- Performs care procedures in accordance with the standards of resident care as set out in the established policies and procedures.
- Follows routine practices in all aspects of care and complies with infection control protocols (i.e. outbreak management).
- Complies with the Occupational Health and Safety Act, Workplace Safety and Insurance Act, and Workplace Hazardous Material Information Systems. This includes recognizing health and safety hazards, reporting incidents and/or any changes to resident’s daily life, fulfilling responsibilities under the applicable legislation, as well as participating in in-services and fire drills. Complies with all relevant corporate policies and procedures.
- Assists in monitoring of specific interventions regularly and offers information or suggestions for improvement.
- Provides ongoing support to co-workers and acts as an advocate for residents and families.
- Assists with other duties to ensure the smooth functioning of the residence.
- Maintains effective communication and good interpersonal relations with residents, families, and other departments.
- Performs duties as assigned by the Health and Wellness Manager.
- Completes all required mandatory training/education.
- Completes documentation of health records accurately and legibly and in compliance with legislations (i.e. Privacy Act).
- Assists in monitoring and evaluating the outcome of specific interventions regularly and offers suggestions for improvement.
- Monitors resident's well being during dining periods.
