Health Access Thorncliffe Park (HATP) is an innovative partnership between Flemingdon Health Centre (FHC) and The Neighbourhood Organization (TNO), and we are seeking a **Community Health Worker** to join our team in a **full-time permanent** role. TNO is a not-for-profit community based multicultural and multi-service agency, located in Toronto, Ontario, with a vision of neighbourhoods where everyone belongs and succeeds.

The Community Health Worker (CHW) will be part of the Community Engagement team responsible for the efficient and effective delivery of programs and services in Thorncliffe Park. The Community Health Worker will work in collaboration with FHC and TNO to deliver an array of programs and services.

The role involves developing educational resources, conducting environmental scans, and identifying collaborations. It involves staying informed about demographics and incorporating Health Promotion best practices. The role also involves maintaining work plans, addressing social conditions, and collaborating with stakeholders.

You will succeed in this position if you have a bachelor's degree in health or social science or an equivalent qualification, at least three years of experience in a community-based environment, and a passion for community health and health promotion. Experience in health promotion, community development, and social determinants of health strategies, and being able to plan, implement, and evaluate programs will be required. You should also have excellent communication skills, experience working with diverse communities, and flexible work hours. Additionally, being proactive in attending community events and outreach activities, cultivating partnerships, and fostering collaboration will be essential. Fluency in prominent languages spoken within the community is considered an advantage.

**Main Duties & Responsibilities**
- Plan, implement and design Community engagement and outreach activities including health and wellness sessions, conduct environmental scans, and foster collaborations.
- Demonstrate innovative outreach by partnering with aligned service providers and the larger community.
- Stay updated on changing demographics and community needs and incorporate sectorial best practices.
- Participate in team meetings, center-wide initiatives, and internal/external committees.
- Monitor and evaluate Community Engagement programs and activities, maintaining budgets and data.

**Screening** - As a condition of employment, the successful applicant must complete a Vulnerable Sector Screening.

**Job Type**: Full-time permanent position

**Schedule**: Monday to Friday, 9 a.m. to 5 p.m. - Some evenings and weekends will be required
