**Role Overview**

The ABI Case Manager works as part of the IST team, to assist persons living with an ABI resulting from traumatic head injury and/or stroke in the community. The ABI Case Manager utilizes his/her knowledge and skills specific to the ABI field in order to perform the following activities:

- Connecting/engaging with clients,
- Developing goal plans with the clients,
- Teaching skills and compensatory strategies to assist clients in living with their ABI
- Supporting individual recovery and assisting clients to link with and/or

**About the Team**
- The multidisciplinary format of our ISTs allow each Case Manager to offer service to clients within his/her area of expertise while promoting knowledge exchange and mutual support amongst team members to enable better care to clients presenting with complex needs. The NY-IST Team is comprised of service providers in the following positions:

- Acquired Brain Injury Case Managers
- Addictions Case Managers
- Dual Diagnosis Case Managers
- Geriatric Mental Health Case Managers Mental Health Case Managers

**Primary Responsibilities**:

- Conduct face-to-face assessments with clients at their locations including but not limited to home, shelters and hospitals.
- Interact directly with referral source, physicians, and other services involved in client’s care.
- Initiate services to clients at the point of intake to ensure that proper assessment and initial service response occurs.
- Develop care plans with clients and complete the Inter Rai Cha assessments as required.
- Collaborate with other systems, organizations and individuals from courts, probation, short-term residential beds program, hospital and community to ensure a well-coordinated service system.
- Record client status/progress using the appropriate electronic charting system.
- A combination of a BSW, MSW or other regulated health care professional designation and a minimum of 1-year direct experience in the delivery of acquired brain injury services,
- OR
- A combination of a post-secondary degree or diploma in a related field with 4 years of direct experience in the delivery of acquired brain injury services.
- AND
- Preference will be given to individuals who have specialized education/training/ certification in the field of acquired brain injury. For example: neuro-rehabilitation, cognitive interventions, neuropsychological assessment, principles of applied behavioral analysis, group facilitation, OBIA’s Brain Basics Training Program, or Level 1 or 2 certification, Behavioural Science Technology.
- Sound clinical knowledge and understanding of the ABI client population and demonstrated ability to work independently and effectively with individuals with serious mental illness and complex needs.
- Knowledge of the principles of case management theory and practice using a client-centred/recovery oriented philosophy.
- Demonstrated ability to use problem-solving techniques to identify and assess client needs and develop appropriate intervention strategies.
- Proven skills in working with challenging behaviors and high-risk environments
- Excellent knowledge of community resources specific to ABI population and demonstrated ability to negotiate with others on behalf of clients.
- Demonstrated initiative and follow-through in completing tasks.
- Excellent clinical judgment and decision-making skills, including a keen awareness of client-clinician boundary issues.
- Demonstrated ability to effectively manage crisis or conflict situations and solid skills in de-escalating crisis situations.
- Ability to function well in team environments and to act as a resource to colleagues.
- High degree of professionalism.
- Excellent communication and interpersonal skills.
- Proficient in MS Office and Inter Rai Cha.
