**Mental Health Crisis Support for African, Black and Caribbean Communities** project provides crisis intervention,
assessment, crisis management and follow-up services to African, Black and Caribbean individuals in the community
who are experiencing mental health and/or substance use crisis and who have negative interaction with the police
as a result of mental health crisis.

Reporting to the program manager, the Family Mental Health Navigator strengthens the role of caregivers of
individuals dealing with mental health crisis, through supportive counseling, education, the promotion of self-care
strategies, assistance with navigating the mental health system and equipping family members to advocate on
behalf of their loved ones.

The Family Mental Health Navigator will offer education and practical skills to help family and friends gain
confidence in their ability to support their loved ones and themselves, and to strengthen their relationships and
networks of support.

**What You Will Do**
Provide direct support services to families and clinical teams including:

- Providing support to families & caregivers during crisis encounters in the inpatient and outpatient and in the

community.
- Assist individuals and families to identify their concerns, increase awareness of and access to resources and

appropriate educational materials.
- Facilitate individual and group counselling sessions for family members.
- Support caregivers in building healthy relationships, enhancing personal connections, implementing self
- care strategies, becoming strong advocates for their loved ones and in developing a sense of belonging and
support in their community.
- Initiate intakes and conduct needs assessments.
- Refer and link clients and caregivers to a range of community resources, including housing, legal supports,

health supports, etc.
- Plan and conduct information sessions in a consultative process with participants and staff.
- Appling culturally safe and agile, trauma informed, relational and person / family
- centered modalities.
- Maintain client and service data and adhere to all reporting requirements and service evaluation timelines.

**What You Bring to the Team**:

- Bachelor’s degree in social work or related field from a recognized university or equivalent combination of

education and experience.
- Experience working with individuals & families experiencing mental health crisis trauma, social isolation

and/or other complex health needs.
- Clinical knowledge and understanding of the client population.
- Strong knowledge of and experience working with African, Black and Caribbean communities and the impact

of intergenerational trauma & understanding.
- Familiarity with system navigation models and principles.
- Knowledge of effective Crisis Intervention techniques and ability to respond appropriately in crisis

situations.
