**Background**

ACSA is a non-profit, multi service agency, addressing needs and empowering children, youth, newcomers, homeless, and otherwise underserved communities. Our core programs include housing and outreach programs, food security programs, newcomer services, youth outreach and therapy programs, child and family programs, early years’ programs, computer access programs, and community engagement.

**Summary of Job Functions**

This project is part of an Initiative to help individuals and households exiting homelessness to transition and stabilize in safe, permanent housing. The Follow-Up Case Manager will provide intensive case management support to 20 individuals referred by the City of Toronto through the Coordinated Access program. Activities will include connection with appropriate services, housing stabilization, eviction prevention and rehousing as necessary. The Case Manager will work within a housing-first, client-centered, and harm reduction approach, in a multi-disciplinary team approach to support clients with complex needs to maintain housing and to be linked with other support services, as determined by each individual client.

**Duties include**:

- Provide case management supports to 20 clients using a strengths-based framework to develop service planning and linkages, goal setting, and supports to stabilize and sustain tenancies.
- Build and maintain responsive relationships with landlords to create positive tenancies, troubleshoot and support the resolution of tenancy issues, providing advocacy and mediation as required.
- Receive and follow up on referrals according to City of Toronto Coordinated Access and ACSA processes; follow all Project processes and requirements; participate in Case Transfer meetings.
- Provide individualized and flexible case management services, through a housing-first approach.
- Assist clients to identify and achieve goals in relation to housing, income supports, substance use, physical and mental health, food security and other service needs as defined by each client.
- Assist clients towards integrating into the community, and empowering their use of resources and services to maximize their independence and potential.
- Provide informed, managed referrals to other services including Rent Bank and legal services, Toronto Employment and Social Services offices, and other agencies.
- Provide harm reduction counselling to identify strategies that reduce negative impacts of substance use on tenancies and make appropriate referrals as necessary.
- Respond to emergency and crisis situations ensuring appropriate conflict resolution, support, debrief and follow up.
- Accompany clients to appointments, including medical and Landlord and Tenant Board hearings as required.
- Work with clients to complete goal setting, community mapping, crisis plans, discharge planning.
- Complete service linkages and referrals to ACSA’s in-house supports and other community resources such as income tax clinic, harm reduction programs, drop-Ins, food banks, voluntary trusteeship, healthcare providers etc. ensuring easier access points to services.
- Maintain accurate case management records, using Pirouette, and complete statistical and qualitative reporting as required by funder and supervisor.
- Attend partner meetings and trainings as set out by the City of Toronto.
- Work collaboratively with case management team and participate in team meetings
- Attend Case Manager/Supervisor meetings
- Perform the responsibilities of the position within the legislative and regulatory standards set out in the applicable Acts. Performs the responsibilities of the position consistent with the policies of ACSA.
- Protect own health and health of others by adopting safe work practices, reporting unsafe conditions immediately. Follows all guidelines for employees and employers as legislated under the Ontario Occupational Health and Safety Act.
- Other duties as required.

**QUALIFICATIONS**:

- Post-Secondary degree in Social Services or related discipline and/or the equivalent combination of education and experience.
- Proven experience providing intensive case management services to clients with complex needs of people who have been homeless
- Demonstrated understanding of housing, health, trauma, substance use, financial, legal,

and other issues that impact clients’ lives.
- Strong understanding of market rent and subsidized housing systems and policies, keeping up-to-date on emerging trends and changes; knowledge of the RTA.
- Demonstrated understanding and commitment to harm reduction framework and housing
- first approach.
- Ability to work independently and as a member of a team.
- Ability to manage issues tactfully,
