**Community Health Worker (2SLGBTQ+ Programs), Health Promotion**
**Permanent Full-time Position: 35 hours per week**
**Expecting Start Date: October 28, 2024**
**Salary Range: $50,718.48 - $60,881.76 per year ($27.87-$33.45 per hour)**
LAMP Community Health Centre is a multi-service, community based charitable organization that provides a wide range of programs and services to improve a person's health. LAMP uses a co-ordinated approach to connect people to medical services and other programs that build personal resources. By providing services that improve physical, emotional, social and economic well-being, people can better control their health and environment.
**Job Summary**
Under the supervision of the Manager of Health Promotion and Community Relations and working within the goals of the health promotion and LAMP CHC programs with a focus on the East Mississauga clientele, the Community Health Worker is responsible for: implementing group support programs for 2SLGBTQ+ adults and their families, providing assistance with system navigation for clients, and acting as a resource and partner in Peel-based, Queer and Trans community development. The work is grounded in a social determinant of health approach; appreciating in particular that social inclusion has a major impact on 2SLGBTQ+ health and wellness.

The incumbent will ensure that all organizational policies are followed.
**Primary Responsibilities**
- With the support of the LAMP CHC Health Promotion team at East Mississauga, responsible for nurturing positive relationships with 2SLGBTQ+ individuals and community partners in the Peel Region.
- Develop and implement social/educational group programs on a weekly basis as co-Facilitator of two ongoing drop-in programs: the QX Social Support group (for 2SLGBTQ+ adults) and Transactivate group (Trans & Gender-diverse adults) and additional 2SLGBTQ+ Family Programs, Gender Journeys sessions and possible other groups as appropriate.
- Plan activities that promote physical health, mental health and wellbeing, social connectedness, individual capacity building and connection to resources in community.
- Provide intake assessment and some case management to 2SLGBTQ+ clients.
- Work in partnership with the Primary Health Care team to support those seeking gender-affirming care living within an expanded catchment area and conduct assessments/ follow-ups as required. Collaborate with the primary care Intake Liaison and Medical Office Assistants (MOAs) on new gender affirming care seeking requests.
- Act as a client advocate on individual and systemic issues, particularly addressing homophobia/transphobia and access to health and social services.
- Lead and foster interprofessional collaboration in support of advancing health equity for 2SLGBTQ+ populations.
- Provide information and referrals to 2SLGBTQ+ adults and their families, and act as a resource to other service providers.
- Conduct community outreach to enhance participation and engagement of priority groups.
- Ensure that relevant individual and group work data are up-to-date and accurate via electronic charting and participate in chart reviews.
- Organize and facilitate 2SLGBTQ+ educational workshops, conferences and community events, including Pride in Peel events along with the communities’ advisory committees.
- Develop and maintain partnerships with community organizations and participate on collaborative tables.
- Propose 2SLGBTQ+ continuous learning opportunities for all staff, including but not limited to Rainbow Health Ontario (RHO) resources.
- Other relevant duties as assigned by the Manager.
**Required Qualifications**
- Undergraduate degree in a relevant discipline from a recognized university or combination of post-secondary education and significant experience in health promotion and/or community development.
- Three to five years of individual and group support work experience in a community setting, preferably with 2SLGBTQ+ populations from diverse communities
- Lived experience working with clients who identify as Trans and Gender-diverse
- An understanding of intersectionality of 2SLGBTQ+ identities and other sociodemographic and cultural identities
- Knowledge of Gender-Affirming Care and of Trans-inclusive resources.
- Strong knowledge of homophobia/transphobia and its impact on 2SLGBTQ+ communities, as well as an understanding of the social determinants of health, the health promotion framework and harm reduction.
- Experience in effective community outreach practices using community development models for diverse racialized populations with intersectional identities.
- Must have strong group facilitation skills and communication skills with a diverse audience
- Positive attitude, empathetic, flexible and dependable with strong initiative and the ability to work both independently and in a team-oriented atmosphere.
