Heartwood Centre for Women is looking for an Indigenous Care Coordinator (ICC) to join our interdisciplinary team. Heartwood is a 90-day inpatient treatment centre for women from across the province who are seeking support for their mental health and substance use recovery.

The Indigenous Care Coordinator walks alongside Indigenous clients during their treatment; offering cultural and spiritual support through one-on-one sessions, group programming and liaising with Indigenous community organizations. This position acts a key resource for Indigenous clients from diverse backgrounds and members of the health care team by advocating for culturally safe care that is holistic and client-centered. The ICC will promote Indigenous cultural safety training and education opportunities to members of the health care team as part of this role.

**What you’ll do**
- Establish effective working relationships with community based agencies and health care providers who provide services for Indigenous communities to facilitate accessible and effective culturally safe, trauma informed client and family care.
- Assess advocate and counsel clients and families concerning emotional and practical problems of moderate complexity; support and promote culturally safe, trauma informed educational and supportive group programs and the development of groups to meet the needs of clients and families.
- Work as a member of the interdisciplinary teams at Red Fish Healing Centre/Heartwood Centre for Women and integrates the role of the Indigenous Care Coordinator into the teams.
- Document client care activities as required.
- Provide liaison services with community agencies.
- Identify clients' and family’s needs for services and makes appropriate referrals, or develop alternate strategies for client support, education and follow-up care.
- Work in collaboration with other disciplines to develop and maintain a list of relevant community referral services and Indigenous community based agencies that may serve our clients.
- Provide a link and liaison services between clients and these agencies.
- Provide information to patients, families and communities regarding the provision and access to culturally safe, trauma informed mental health and substance use care programs and services.
- Coordinate client/family spiritual care requests and/or access to sacred spaces within the red Fish Healing Centre/Heartwood Centre to support Indigenous practices, beliefs and customs.

**What you bring**

**Qualifications**
- Bachelor’s degree in a relevant Social Sciences discipline.
- Two (2) years related experience working in a health care environment that includes experience counselling individuals and families and group work or an equivalent combination of education, training and experience.
- Comprehensive understanding and experience delivering culturally safe, trauma informed care
- Eligible for registration with designated professional regulatory body.

**Skills and Knowledge**:

- Knowledge of federal, provincial, territorial, regional and Indigenous organizations mandated to provide mental health and substance use care services.
- Extensive knowledge of health issues impacting Indigenous communities.
- Extensive knowledge of community-based Indigenous organizations across BC.
- Interpersonal skills including the ability to communicate effectively with clients, families, staff and the public.
- Ability to work independently and co-operatively as part of an interdisciplinary team.
- Ability to communicate effectively both verbally and in writing.
- Ability to deal with others tactfully and with demonstrated sensitivity.
- Ability to organize and problem solve.
- Ability to assess, plan, implement and evaluate.
- Awareness of and commitment to learning and understanding the Truth & Reconciliation Commission’s 94 Calls to Action (2015), In Plain Sight Report (2020), BC's Declaration on the Rights of Indigenous Peoples Act (2019), and other related reports.
- Commitment to upholding the shared responsibility of creating lasting and meaningful reconciliation in Canada as per TRC (2015) and BC's Declaration on the Rights of Indigenous Peoples Act (2019).
