**Job Summary**

The Case Worker provides psychosocial assessment and a range of interventions to assist people living with HIV/Hepatitis C. The ultimate goal of the Case Worker is to help individuals cope with their chronic illness, social challenges and side effects of treatment programs. Interventions include consultation, counseling, education and advocacy functions for individuals and their families. The Case Worker takes a multidisciplinary approach to support and will draw on team members as deemed appropriate. The Case Worker will also provide education internally to staff/volunteers

**Key Responsibilities**
- Ensures that appropriate treatment plans are developed, implemented and evaluated
- Monitors client progress including evaluation of, and adjustments, to treatment plans
- Ensure case notes are available in a manner that meets professional standards of practice
- Maintain a supportive relationships with the client and family members
- Participates as an active member of the multidisciplinary team in regular client conferences
- Acts as a clinical resource consultant to staff regarding care issues and participates with the multidisciplinary team in care planning
- Participates on committees, in working groups
- Maintain current knowledge of community resources pertinent to the particular client base and geographical area in which clients reside
- Ongoing participation in professional activities that will enhance and develop personal growth

**Education & Training**
- MSW/HBSW or SSW or undergraduate degree/certificate in a related field at an accredited institution **_or_** relevant workplace experience.
- At least three years of relevant experience in assessment and treatment of individuals with chronic disease and demonstrated familiarity with issues relating to mental health and addictions
- Comprehensive knowledge and experience in HIV/Hepatitis C, harm reduction, addictions and mental health
- Strong knowledge of issues affecting marginalized communities
- Knowledge of policies, legislation, programs, and issues related to area of practice
- Valid Driver’s License
- Must be able to pass Vulnerable Persons Criminal Reference Check

**Experience**
- Experience in working in a community setting
- Facility in a second language an asset

**Salary**: From $25.67 per hour
