**KEY FUNCTION**:
Provide advanced clinical practice guidance and consultation for the care, management, and rehabilitation of adult patients with an Acquired Brain Injury (ABI) across the Continuum of Care (CoC) at HSN and across the region. Facilitate, promote, and lead process and quality improvement initiatives to maintain ABI best practice, and HSN’s Strategic Direction Address the training and education needs of hospital-based ABI service providers.
**REPORTING**:
Under the general direction of the Clinical Manager, Intensive Rehabilitation Unit.
**DUTIES**:
- Provide leadership to program and corporate initiatives, health care and rehabilitation planning, and policy development within HSN, and across the North East (NE) and Ontario to integrate best practice, education, and research for optimal patient and family care.
- Work with patient care teams to assess compliance and implement best practice care and rehabilitation of the ABI patient population.
- Develop communication links with hospital and community providers across the NE region and the province to collaborate and meet the needs of patients with ABI.
- Develop good working relationships with academic and research communities to enable opportunities for collaborative research that can benefit HSN’s ABI programs and the region’s advancements in ABI care (e.g. Ontario Neurotrauma Foundation (ONF), Greater Toronto Area (GTA) Rehab Network, and Northern Ontario School of Medicine (NOSM)).
- Consult with Emergency personnel and other Inpatient/Outpatient units within HSN and throughout the region to facilitate the flow of service for ABI patients.
- Act as a liaison for referrals to HSN’s Regional Inpatient and Outpatient ABI Services and other hospital and community-based rehabilitation programs when appropriate.
- Act as an ABI Best Practice Clearinghouse and disseminate new ABI research, study findings, and evidence/consensus-based guidelines to all relevant health care professionals.
- Work with relevant partners to identify and address ABI education and training needs of HSN staff and regional stakeholders; leverage cost-effective opportunities to bring ABI education to the North via Webinars, meetings, and the Ontario Telemedicine Network (OTN); Facilitate education events.
- Consult, educate, and train on the assessment, treatment, and management of ABI to Health Care Professionals and service providers; escalate duties as required.
- Develop, update, and revise patient and family ABI education materials as needed.
- Chair the HSN ABI Continuum of Care Steering Committee.
- Determine and align improvement projects with HSN’s Strategic Plan; monitor and adjust to achieve goal outcomes.
- Participate in education and training specific to current, relevant federal and provincial health and safety legislation, standards and guidelines.
- Educate and promote health, safety and wellness in the work place.
- Represent the department or program on various committees and in meetings as required.
- Perform other duties as required.
**QUALIFICATIONS**
**EDUCATION AND TRAINING**:
- Minimum of a Master’s Degree in Nursing, Rehabilitation, or in a health-related field (e.g. Physiotherapy, Occupational Therapy, Social Work, Registered Dietitian, and Speech Language Pathology), from an accredited university.
- Current Certificate of Registration as a Regulated Health Professional with a membership in good standing with a provincial College or governing body.
- Ministry of Labour “Worker Health and Safety Awareness in 4 Steps” training certificate is required.
**EXPERIENCE**:
- Minimum of five (5) years’ experience working in an Intensive Rehabilitation department for adults with mild to severe ABI (e.g. traumatic brain injury) within a health care environment.
- Training or recent experience with organizational development, adult education, knowledge transfers, and project management is preferred.

- Demonstrated leadership, networking, and conflict resolution skills.
- Demonstrated excellent inter-professional competencies with the ability to promote and facilitate teamwork across HSN.

- Demonstrated training, experience or utilization of lean methodology for process improvement.
- Demonstrated ability to independently identify issues, plan improvements, measure success and continue improvement.
