**Purpose**:
Under the direction of the Program Manager, the Forensic Supportive Housing Program (FSHP) Housing Based Case Manager will work with the Transitional Case Manager, Housing Success Team, TRHP, and the Forensic team at the North Bay Regional Health
Centre to provide long-term supports designed to help forensic participants remain stably housed while still meeting the requirements of the Ontario Review Board. This position will work with the individuals within their homes and other community settings in order to mitigate the risk of individuals breaching their ORB dispositions and being re-hospitalized.

**Education and training**:

- Proof of successful completion of a post-secondary education in Social Services or related field

**Requirements**:

- A current negative Police Vulnerable Sector Check
- A valid Class G driver’s license and proof of insurance
- Access to a reliable vehicle daily for work purposes
- Current CPR certificate - level C or willingness to obtain
- Current Non-Violent Crisis Intervention (NVCI) training or willingness to obtain
- Current Applied Suicide Intervention Skills Training (ASIST) or willingness to obtain
- Current Safe Food Handling course or willingness to obtain
- Able to lift at a minimum 20 lbs. from squatting to standing position
- Phone and on-line service support experience is an asset
- **Bilingualism (English/French) is required**

**Experience**:

- Minimum 3 to 5 years of experience working with vulnerable populations, specifically adults living with a serious and persistent mental illness and/or acquired brain injury (ABI) and/or Addictions and/or complex healthcare needs providing assessments and rehabilitation services
- Demonstrated knowledge of, and skills working in, the mental health field, forensics, Ontario Review Boards, psychosocial assessments, crisis intervention, community resources and the criminal justice system
- Experience working in a community setting and working knowledge of community resources

**Knowledge/Skills/Abilities**:

- Basic working knowledge of computer software
- Ability to work cooperatively and independently with mínimal supervision and as part of a team
- Effective problem solving and decision-making skills
- Proficiency in managing challenging behaviours
- Excellent interpersonal/verbal/written communication skills
- Ability to maintain confidentiality as per PHIPA (Personal Health Information Protection Act)
- Working knowledge of the Mental Health Justice System, Mental Health Act, Residential Tenancy Act and the Health Care

Consent Act
- Knowledge/experience using the principles and practices of psychosocial rehabilitation
- Strong knowledge of mental illness, forensics, crisis intervention, life skills training, and knowledge of community resources
- Ability to provide education to clients and family members to better understand and cope with their mental illness including medication issues and relapse prevention
