SOMMAIRE: Le rôle de responsable de cas est de fournir et de coordonner les services psychosociaux et concrets aux personnes âgées et/ou aux personnes âgées vulnérables de la communauté juive dans le but d’améliorer leur fonctionnement, leur autonomie et leur intégration sociale.

TÂCHES ET RESPONSABILITÉS:
▪ Évaluation psychosociale et/ou financière des clients

▪ Réalisation d’une évaluation complète des soins à domicile en utilisant les outils désignés et en effectuant une évaluation à domicile

▪ Élaboration et mise en œuvre du plan d’intervention en consultation avec le client et/ou le soignant et tout autre partenaire professionnel ou communautaire pertinent

▪ Responsable du suivi régulier, du soutien psychosocial continu et de la réévaluation des besoins du client, y compris intervention en cas de crise, intervention de l’organisme, recommandations, présentation des à des comités internes et/ou externes

▪ Maintenir une communication continue avec l’équipe de soins à domicile

▪ Consultation et collaboration avec divers professionnels et systèmes internes et externes

▪ Identification des tendances des demandes et des besoins de la communauté

▪ Participation à des ateliers de formation et d’éducation

▪ Satisfaction de toutes les exigences administratives et de données

QUALIFICATIONS & COMPÉTENCES:
▪ Base solide de connaissances et de compétences cliniques

▪ Excellentes compétences d’écoute et de communication

▪ Capacité de travailler en équipe

▪ Fortes compétences organisationnelles et administratives

EXIGENCES:
▪ Bac en travail social ou dans un domaine connexe

▪ Minimum de 2 ans d’expérience de travail avec des personnes âgées

▪ La connaissance du russe est un atout majeur.

▪ Organisé, collaboratif, travail bien avec les autres équipes

▪ Connaissance de la communauté juive sera un atout

▪ Compétence avec Logiciel Microsoft; Excel, Word, et Outlook

JOB SUMMARY

The case manager’s role is to provide and coordinate psychosocial and concrete services to seniors and/or vulnerable older adults of the Jewish community with the goal of enhancing their functioning, autonomy and social integration.

DUTIES AND RESPONSIBILITIES

▪ Psychosocial and/or financial assessment of clients

▪ Completion of full Homecare assessment by using the designated tools and conducting in-home evaluation

▪ Development and implementation of intervention plan in consultation with client and/or caregiver and any other relevant professional or community partner

▪ Responsible for regular follow-up, ongoing psychosocial support and reassessment of client’s needs including crisis intervention, agency response, referrals, advocacy, case presentation to internal and/or external committees

▪ Maintaining ongoing communication with Homecare team

▪ Consultation and collaboration with diverse internal and external professionals and systems

▪ Identification of trends in requests and community needs

▪ Participation in training and educational workshops

▪ Fulfilment of all administrative and data requirements

SKILLS & COMPETENCIES

▪ Solid base of clinical knowledge and skills

▪ Excellent listening and communication skills

▪ Ability to work in a team

▪ Strong organizational and administrative skills

**REQUIREMENTS**:
▪ Bachelor’s degree in Social Work or a related field

▪ Minimum of 2 years of experience working with older adults

▪ Russian speaking a strong Asset

▪ Knowledge of the Jewish Community a strong asset

▪ Organized, collaborative, works well with other teams.

▪ Competency in Excel, Word, PowerPoint & Outlook
