Job Description
In this valuable role, the Clinical Supervisor is accountable for the supervision of PSW’s and HSW’s who are caring and making a difference for clients within the community. The Clinical UCP Supervisor will have the expertise to assess, implement and evaluate a wide range of clinical care interventions to ensure the delivery of safe, quality, and ethical care. Respond to and advise/resolve concerns from community staff, patients/families, and other team members. Work collaboratively with nursing team members to support clinical documentation and submission requirements according to specific funder reporting guidelines. Engage patients in problem solving using a collaborative approach, offering health teaching, building rapport and trust to identify patient health care needs, strengths, capacities, and goals. Engage staff in problem solving, offering education, coaching, and mentoring in resolution of problems. Current RN or RPN registration with the College of Nurses of Ontario and entitled to practice without restrictions Minimum of 5 years’ experience as a practicing Registered Nurse or Registered Practical Nurse Comprehensive patient health assessment skills are required Possess an understanding of the principles of community nursing practice Proven experience leading and managing a team Commitment to professional development in clinical practice that is relevant to home health care A. certification as a Community Health Nurse (CCHN(C)) is considered an asset this is a replacement position Time Type Full time ParaMed is Canada's leading provider of home health care services, driven by a mission to provide people with the care they need, wherever they call home.
