- Conduct assessments and oversee service coordination
- Provide individualized and group counseling
- Collaborate to implement resident care plans
- Deliver crisis intervention and short-term solutions
- Contribute to policy development on social work services
- Bachelor’s degree in Social Work and relevant registration
- At least 2 years of experience in long-term care
- Proficiency in relevant software applications
- Empathetic approach to resident care
- Valid Police Vulnerable Sector Screen
