Elevate patient care as a Nurse Practitioner in Halifax. Support rehabilitation efforts and program development while leading adults from acute care to home-based recovery. In this pivotal role, you will assess patients independently and collaborate with a multidisciplinary team, including Nursing and Rehabilitation experts. You'll design evidence-based care plans tailored to individual patient goals, ensuring seamless transitions from hospital to home. Contribute to innovative quality initiatives that enhance transitional healthcare programs. Key Responsibilities:
- Assess and manage patient care independently
- Prescribe medications and perform clinical procedures
- Collaborate with care teams for safe hospital discharges
- Develop personalized care plans with patients and families
- Drive quality improvement initiatives in transitional care
- Master’s degree in Nursing and Nurse Practitioner Certification
- Good standing with local Nursing college
- Experience with vulnerable populations is advantageous
- Passion for supporting patient independence and wellness
- Fluent in English
