For more information, visit www.camh.ca. Responsibilities Act as the point of contact for referral management for Access CAMH, liaising with referral sources and partner hospitals/agencies. Provide clinical leadership to Access CAMH, ensuring alignment with provincial standards and stakeholders, in collaboration with the manager. Lead and/or participate in provincial and regional committees focused on continuous quality improvement to optimize referral management. Provide clinical direction and support to the team to ensure timely and effective triaging, screening, and service matching. Provide coaching, education, and mentorship to all staff in the program to meet best practice standards. Engage the clinical team and external service providers for patients who need enhanced care planning or service resolution. Serve as a clinical resource to staff and other health professionals regarding service coordination/access issues. Coordinate and communicate with external stakeholders (Toronto Region hospitals, mental health organizations, primary care, other provincial hubs) to facilitate efficient referral flow. Build partnership and collaboration with system partners within Toronto Region and across the province to build and standardize care pathways. Support leadership at external organizations to align with provincial standards for coordinated access. Support staff recruitment and onboarding, in collaboration with the manager. Provide and/or coordinate education/training opportunities for all staff in collaboration with the manager. Initiate, lead and facilitate quality improvement projects and huddles within the team and across system partners. Participate in the development of a program quality scorecard aligned to corporate or portfolio requirements. Monitor key program performance indicators and implement necessary engagement and process changes to optimize referral management efficiencies. Monitor safety issues, complaints and implement strategies to address them using a quality improvement approach. Conduct audits related to quality-of-care issues, report outcomes, and collaborate on strategies for ongoing improvement. Promptly follow up on any patient complaints or incidents, completing incident reports and ensuring satisfactory resolution. Communicate staffing utilization or issues that impact program operation to management. Requirements Master’s Degree. LEAN, Six Sigma or Quality Improvement certificate. Registered with an Ontario regulatory college (e.g., COTO, OCSWSSW). Minimum 5‑7 years of mental health and addiction service delivery experience in hospital and community settings. Minimum 3 years of experience in a Mental Health and Addiction coordinated access model. Minimum 3‑5 years leading clinical quality improvement projects relevant to mental health and addiction services. Minimum 3 years of mentorship, supervision or teaching experience/qualifications. Significant experience leading knowledge translation or change management initiatives to improve care/service delivery. Excellent representational skills at internal and external committees. Ability to work across multiple sectors, building positive relationships with partners. Strong skills in change management and conflict resolution. Deep understanding of the mental health and addiction system and services in the region.
