Responsibilities:

Clinical and Medication Management:

Make a meaningful difference in patients' lives by providing timely access to medications, optimizing therapy regimens, and promoting medication safety and adherence to improve health outcomes.

Conduct comprehensive patient assessments, including medical history reviews, physical examinations, and diagnostic tests, to identify health conditions and treatment needs.

Prescribe medications, including initiating, adjusting, and renewing prescriptions, within the scope of your APA and in accordance with relevant regulations and guidelines.

Collaborate with healthcare providers, including physicians, nurse practitioners, and other members of the healthcare team, to coordinate patient care and optimize treatment plans.

Provide patient education and counselling on medication use, potential side effects, drug interactions, and lifestyle modifications to promote medication adherence and enhance therapeutic outcomes.

Monitor patient responses to treatment, conduct follow-up assessments, and make appropriate adjustments to medication regimens as needed to achieve desired health outcomes.

Chronic Disease Management:

Apply medication management principles consistently.

Initiate, modify, and monitor drug therapy for chronic diseases.

Educate patients and healthcare professionals on disease management.

Health Promotion/Screening:

Promote healthy living and support immunization programs.

Identify health risks and educate at-risk individuals.

Patient Self-Care:

Assist patients in selecting appropriate self-care regimens.

Assess treatment options considering the patient's health condition.

Ensure the safe preparation and handling of medications.

Document incidents and report adverse drug reactions.

Leadership:

Act as a professional leader and supervise pharmacy staff.

Assist with performance evaluation and inventory processes.

Education and Experience:

Bachelor’s degree in pharmacy and valid licensure.

Additional Prescribing Authority or willingness to attain.

Valid Injection Certification or willingness to attain.

Qualifications and Skills:

Proficiency in pharmacy software and database programs.

Strong interpersonal, communication, and customer service skills.

Excellent organizational and time management skills.

Takes initiative and exercises independent judgment.

Remains current with advances in health and pharmacy-related matters.
