Clinical Pharmacist, Primary Care

In accordance with established mission, vision, values and strategic priorities of the Northern Health and reporting to the Pharmacy Operations Lead, Primary and Long-Term Care, this position provides clinical pharmacy services to patients of the primary care network. As a member of an interprofessional team this position works directly with patients to identify and resolve actual and potential drug therapy problems through provision of comprehensive medication management. Shift Rotation/Hours of work: Monday to Friday, Days – 08:30 to 16:30. Rotation and days of work are subject to change based on operational needs. Position serves Prince Rupert and Coast Tsimshian Prince Rupert Regional Hospital is a 24‑bed facility that sees an average of 22,000 ER visits per year. It is a fully equipped facility with Medical Imaging, Laboratory, ICU, ER and Maternity. Services include: Acute Care Internal medicine Internal Medicine Ultrasound CT scans Surgery Emergency department Extended care Pediatrics Radiology Obstetrics Gynecology Additional services such as Diabetes Education, Healthy Heart and Rehabilitation, Podiatry, Orthopedics, and other visiting specialists. A valid Class 5 drivers' licence and access to a personal vehicle for business related purposes.
