Key Clinical Services Individualized pharmacotherapy Counselling patients about their medication therapy Work in collaboration with physicians and other health care providers to develop, implement, and monitor therapeutic plans Assist in the development of drug therapy guidelines for individual disease conditions Prepare and submit case studies and articles for publication in professional journals Act as department resource person within your assigned area Participate in medication use evaluations, quality improvement initiatives, and clinical drug trials Provide mentorship and supervision to pharmacy students, residents, and new staff Ensure prescriptions are validated and dispensed accurately; resolve medication safety concerns and discrepancies expediently, documenting professional practice activities proficiently Does this sound like you? Detailed Overview Situated within identified Correctional Health Services locales, this position provides clinical pharmacy services to clients of the Correctional Health Services. As a member of an interprofessional team, this position works directly with clients to identify and resolve actual and potential drug therapy problems through the provision of comprehensive medication management. Responsibilities Identify and resolve drug therapy problems in collaboration with the client and other health care team members Provide comprehensive medication management and pharmaceutical care by monitoring effectiveness, adherence, adverse reactions, drug interactions, and laboratory incompatibilities; communicate findings, recommendations, and plans to clients and practitioners Consult with clients and team members to develop client‑specific therapeutic plans based on chart review and clinical assessment Maintain documentation in the health record per organizational standards Use appropriate resources (e.g., provincial programs, academic detailing, optimal drug use initiatives, opioid stewardship, de‑prescribing) to support professional practice and patient care Establish linkages with health authority, community pharmacy services and other relevant community services to support continuity of care for clients Provide drug information, consultation, and other support services to medical, nursing, pharmacy, and other health team members regarding drug‑related issues, questions, and client‑specific regimens Conduct client counseling and education regarding drug therapy Provide in‑service education to the health care team where appropriate Provide instruction and supervision to pharmacy undergraduates and residents Participate in committees to offer guidance and advice on drug therapy Participate in quality improvement activities by reviewing practice, identifying problems, and recommending and evaluating modifications Maintain workload records and metrics as required Participate in programs such as Patient Safety and Drug Reaction Reporting by collecting and compiling relevant data Share the responsibility for orienting new personnel and act as a role model Maintain a working knowledge of PharmaCare policies and procedures Perform other related duties as assigned Qualifications Education and Experience Current registration as a pharmacist with the College of Pharmacists of British Columbia Two (2) years of recent, relevant patient care experience or an equivalent combination of education, training, and experience Valid BC Driver's Licence and access to a personal vehicle for business‑related purposes Skills and Abilities Broad knowledge of organization policies, procedures, and standards of care Knowledge of evidence‑based clinical pharmacy practice Knowledge of medication use evaluation Knowledge of research processes and methodology Knowledge of drug distribution processes in a health system setting Ability to apply clinical pharmacy knowledge and make decisions with minimum supervision Ability to communicate effectively, both verbally and in writing Ability to establish and maintain effective working relationships with other health practitioners, clients, family members, and the public Ability to organize and prioritize work Ability to operate related equipment Physical ability to carry out the duties of the position About Fraser Health Fraser Health is the heart of health care for over two million people in Metro Vancouver and the Fraser Valley in British Columbia, Canada, on the traditional, ancestral and unceded lands of the Coast Salish and Nlaka’pamux Nations and is home to 32 First Nations within the Fraser Salish region.
