Pharmacist – Surrey Memorial Hospital Part‑time relief position available for a Pharmacist at Surrey Memorial Hospital in Surrey, British Columbia. Detailed Overview The Pharmacist provides distribution pharmacy services, performing duties such as interpreting medication orders/prescriptions, assessing prescriptions for therapeutic suitability, ensuring medications are efficiently and accurately dispensed or compounded, and maintaining complete dispensing records. The Pharmacist communicates with physicians, patients, and other healthcare team members for prescription clarification and problem resolution, provides instructions on proper use/administration, storage, and disposal of medications, participates in data collection for medication use evaluation projects, implements dispensary‑based medication safety initiatives, and contributes to internal publications and clinical drug trials. Additionally, the Pharmacist functions as a Clinical Pharmacist, resolving select patient drug‑related problems and participating in nursing ward rounds and program team meetings while documenting patient‑specific therapeutic plans. Responsibilities Interpret medication orders/prescriptions and dispense medications using a computerized medication information system in accordance with legal, professional, and departmental policies, procedures, and standards. Assess prescriptions for therapeutic suitability by referencing standardized drug information texts, laboratory data, and patient demographics. Review patient profiles for allergies, potential drug interactions, therapeutic duplications, or contraindications, and withhold medications until safety concerns are resolved. Communicate with physicians, patients, and other healthcare team members for prescription clarification and problem resolution, documenting outcomes. Provide instructions on proper use, administration, storage, disposal, and potential side effects of medications, both verbally and in writing to healthcare professionals and patients. Assist with medication reconciliation and verify allergy information during medication history acquisition. Ensure accurate dispensing or compounding, labeling, and completion of dispensing records. Provide medication counselling to patients and families, discharge counselling for appropriate patients, and document interactions in the patient health record. Review screening reports or targeted drug lists to identify potential drug‑related problems using laboratory data. Provide basic pharmacokinetic interpretations for patients receiving targeted drugs such as aminoglycosides, warfarin, and vancomycin. Collect data for medication use evaluation projects, maintain inventories, requisition drugs and supplies, monitor expiry dates, report procedural errors, and participate in quality improvement activities. Implement dispensary‑based medication safety initiatives and quality control activities according to established standards. Maintain records for narcotic and controlled drugs, special access program drugs, and departmental statistics per legal and departmental standards. Orient pharmacy students, residents, technicians, assistants, and new staff on dispensary functions, procedures, and equipment, providing guidance and progress feedback. Contribute to internal publications and conduct clinical drug trials, reviewing adverse reaction cases and completing documentation. Participate in staff meetings, committees, and other programs, identify opportunities to improve processes and procedures, and refer to supervisors or others. Act as a clinical pharmacist, resolve select patient drug‑related problems, and participate in ward rounds or program team meetings documenting therapeutic plans. Perform other related duties as assigned. Qualifications Education and Experience Current registration as a Pharmacist with the College of Pharmacists of British Columbia. Skills and Abilities Knowledge of drug distribution processes in a hospital setting. Ability to communicate effectively both verbally and in writing. Ability to establish and maintain effective working relationships with other healthcare practitioners, patients, family members, and the public. Ability to organize and prioritize work. Ability to operate related equipment. Physical ability to carry out the duties of the position.
