Reporting to the Project Director, Homelessness Initiatives and Prevention Services in the Toronto Shelter & Support Services (TSSS) Division, the Project Manager TSSS will work with our allied health partners to create referral pathways between primary, mental health, emergent, and community healthcare and shelter providers, which in turn will result in improved, sustainable health and housing outcomes for individuals experiencing homelessness across the city. The successful incumbent will support Divisional priorities by providing leadership and expert guidance on organizational initiatives to resolve care access barriers across the Health Care System. Major Responsibilities include, but are not limited to: Cultivates and retains strategic relationships with corporate representatives, allied healthcare partners, politicians from three levels of government and internal senior level management to address system level gaps in health care access. Leads and supports the development of cross-sectoral agreements, medical directives (led by the health provider), policies, or protocols to improve access to and provision of healthcare for individuals experiencing homelessness. Negotiates and implements innovative solutions to complex challenges involving multiple stakeholders to increase access to healthcare for individuals experiencing homelessness. Represents the division in meetings with a wide and diverse range of internal and external healthcare involved stakeholders (i.e. City Divisions, community groups, agencies, senior staff, and elected officials). Advocates at a systems level to improve health outcomes for the diversity of communities and individuals experiencing homelessness (race, gender, status, income levels, unhoused). Supports the ongoing evaluation and implementation of the Homelessness Health Services Framework by highlighting barriers to care for individuals experiencing homelessness. Develops and implements detailed plans to support new program and system changes across the Division and relevant stakeholders. Prepares project reports, staff reports, briefing notes, and other status update documents for senior leadership. Anticipates, analyzes and identifies organizational and program delivery impacts of emerging issues and activities to recommend innovative solutions. Leads the implementation and management of data collection strategies and planning tools to support the development of innovative solutions to healthcare access for those experiencing homelessness. Creates a chain of knowledge transfer to empower site level leadership to navigate referral pathways to healthcare. Leads working/advisory groups in project planning and program development processes by identifying, formulating and recommending new program features, and preparing and supporting the delivery of detailed implementation plans. Supervises, motivates and collaborates with project team members, ensuring effective teamwork, high standards of work quality and organizational performance, continuous learning and encouraging innovation in others. Are you the ideal candidate we are looking for? Post-secondary education in a relevant discipline (e.g. health, social sciences, public administration, etc.) or an equivalent combination of education and relevant experience. Considerable experience establishing, building and sustaining strategic organizational and intergovernmental partnerships to assess and adapt programs to meet the needs of diverse and marginalized communities. Considerable experience in large-scale project management within the health and human or social services sectors, including administration, service planning, development of logic models, service delivery, policy/program development and analysis, program evaluation, and/or community development. Considerable experience in advisory and consultation processes involving councillors, staff, external partners and community members; including the ability to maintain effective working relationships. Considerable experience managing, leading and motivating a team of professionals, while supporting organizational objectives. Ability to achieve alignment and support from Senior Leadership on new approaches, change initiatives and implementation of health-related best practices. Skilled in writing complex reports (e.g. briefing notes, staff reports, etc.) for senior leadership. Highly developed communication, presentation, and human relations skills with the ability to communicate persuasively and effectively at all organizational and intergovernmental levels. Political acumen, judgment and insight, and conflict resolution including the ability to negotiate and achieve consensus among parties having different objectives. Ability to lead change management initiatives with the goal of creating a progressive organizational culture that supports effective program delivery. Highly developed strategic, organizational, analytical and conceptual thinking skills with the capacity to understand complex operating and policy issues, performance measures, and the ability to respond to emerging issues. Current knowledge of health inequities and social determinants of health. Knowledge of relevant issues and trends as related to housing and homelessness. Knowledge of collective agreements, City guidelines, policies, applicable statutes, municipal by-laws, and relevant legislation including Employment Standards Act, Human Rights Code and Occupational Health & Safety Act.
