Membership, in good standing, with the applicable regulatory body:College of Nurses of Ontario (RN)College of Physiotherapists of OntarioCollege of Occupational Therapists of OntarioOntario College of Social Workers and Social Service Workers (RSW)2+ years of recent experience in community health or a related fieldKnowledge of the health care delivery system and community resourcesExcellent interpersonal, communication, assessment, problem‑solving, and decision‑making skillsEffective time management, prioritization and organizational skills, with the ability to work independently and co‑operatively in a busy multidisciplinary environmentEstablished ability to accurately complete required documentation, reports and formsA valid driver’s licence and access to a reliable vehicleProficient in a Windows environmentWe have a mandatory COVID‑19 vaccination policy. Position location and travel Regular travel within the geographical region of Ontario Health atHome Central East may be required.
