Community Health Nurse - Registered Nurse - Tri-CitiesWe are currently looking to fill a Casual opportunity for a Community Health Registered Nurse with Home Health located in Tri-Cities, B.C.Salary The salary range for this position is CAD $41.42 - $59.52 / hourOverviewIn accordance with the British Columbia College of Nurses and Midwives (BCCNM) standards of practice and the Mission and Values of Fraser Health, the Community Health Nurse (CHN) - Registered Nurse works independently in the community setting. Works collaboratively as a member of an interprofessional team in the management of an assigned client caseload including assessments, coaching, interventions, client care services and follow up to enable clients and their families to live confidently and safely at home and/or in the community.This role emphasizes the promotion, maintenance and restoration of health through teaching, counselling and direct client care; facilitates and manages client transitions across the healthcare continuum to optimize recovery and minimize avoidable admissions to residential and/or acute care facilities; collaborates and ensures linkages with acute, primary and community care providers and family/supports regarding client care planning and transitions to primary/community care.ResponsibilitiesEstablishes a therapeutic relationship with the client to support autonomy in decision-making and care planning, respecting dignity and privacy.Screen referrals, perform individualized client assessments, develop interprofessional care plans, and provide clinical care and referrals for clients with multiple complex chronic conditions; coordinate with the primary care provider, family/supports, other health professionals, clinics, hospitals and community resources.Develops a comprehensive shared patient/client care plan in collaboration with the interprofessional team; facilitates the transition of the care plan to the referring source and to community providers.Provide direct client care and identify additional services required; explain care to clients and families as appropriate.As required, assign direct client care tasks to Community Health Workers and, when appropriate, delegate tasks; provide in-home demonstrations and training to standards of practice; monitor CHWs and collaborate with the CHN-LPN for task performance.Facilitate care conferences to review client care plans; monitor and evaluate the care plan to meet client goals and develop next steps with the client and family.Discuss clinical findings with client/family to develop action plans and goals; access and review client data and integrate findings into the care plan; set priorities and respond to changing needs.Support clients and families before, during and after interprofessional conferences and clinic visits; advocate for client choices and provide direct care during transitions to other programs or providers.Arrange and participate in joint home visits with other healthcare professionals; advise on available community resources.Provide health counseling and education to support self-management, wellness promotion, and prevention through clinic, telephone, or home visits; collaborate to facilitate information flow within the primary & community care network.Maintain clinical records and statistics in accordance with policies and procedures.Maintain professional practice growth by staying current with standards of practice and participating in ongoing education.Participate in department quality improvement, risk management, and, as required, research opportunities.Participate in orientation and ongoing education of nursing staff and students as a preceptor when appropriate.Contribute to development and revision of standards of care, policies and procedures and support quality improvement initiatives; provide recommendations to management.Perform other related duties as assigned.QualificationsEducation and ExperienceCurrent practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM). One (1) year of recent related clinical experience assessing and treating complex geriatric and/or adult patients with chronic health conditions in an acute or community/outpatient setting, including recent care and discharge planning, or an equivalent combination of education, training and experience. Valid BC Driver's license and access to a personal vehicle for business-related purposes.Skills And AbilitiesDemonstrated knowledge and competence in gerontology and adults living with complex frailty and chronic illnesses; ability to complete initial and ongoing client assessments and provide nursing care; strong communication skills (verbal and written); ability to synthesize data from multiple sources; knowledge of broad health care services and community resources; client-focused care with sensitivity to diverse cultures; ability to manage and prioritize diverse health issues; ability to teach clients and others about health care concepts; ability to work independently and as part of an interprofessional team; ability to operate related software and equipment; physical ability to perform duties.About Fraser HealthFraser Health is the heart of health care for over two million people in Metro Vancouver and the Fraser Valley in British Columbia.
