Role Description This is a full-time, on-site role for a Nurse Practitioner at our facility located in Brantford, ON. The Nurse Practitioner will provide comprehensive primary healthcare services, including diagnosing and managing acute and chronic conditions, prescribing medications, and performing therapeutic procedures. The role involves collaborating with multidisciplinary teams to develop and implement care plans, ensuring high standards of quality and patient-centered care. The Nurse Practitioner will also participate in health promotion, patient education, and quality improvement initiatives, fostering a caring environment for each resident. Qualifications Clinical skills in diagnosing and managing acute and chronic conditions, along with experience in prescribing medications and performing therapeutic procedures Strong communication, interpersonal, and collaborative skills for working with residents, families, and interdisciplinary healthcare teams Knowledge of patient education, health promotion, and preventive care practices Experience in long-term care settings or with geriatric populations is highly advantageous Current Nurse Practitioner certification and licensure in Ontario, with a strong commitment to continuing education and professional development Ability to maintain accurate medical records, documentation, and adhere to all relevant healthcare regulations and ethical standards #J-18808-Ljbffr
