The Nurse Practitioner will be an integral part of a collaborative and inter-disciplinary team focused on delivering exemplary care to our residents. The primary role of the Nurse Practitioner is to direct, provide and evaluate holistic patient care for a specified patient population. The Nurse Practitioner will provide leadership to promote program development process improvements as well as the enhancement of professional standards within the Clinical Team at SJLCB. The Nurse Practitioner will act as a clinical resource for nurses and other Regulated Health Care Professionals.Key Accountabilities & Success CriteriaCompletes comprehensive health history and physical assessmentIdentifies and orders diagnostic tests and procedure according to approved medical directives within the NP scope of practiceCollects, interprets and analyzes data from multiple sources and uses this data to formulate medical diagnoses within scope of practiceDevelops implements and documents the plan of care for the patientWorks collaboratively with patients, physicians and other members of the interdisciplinary team to develop a plan of carePrescribes pharmacological and non-pharmacological therapies according to approved medical directives.Evaluates and revises plan of care based on patient responsePerforms skills and invasive/non-invasive procedures related within scopeInitiates interventions for the purpose of stabilizing patients in urgent and emergent situationsDevelops cohesive working relationships with interdisciplinary providers in our community, both internally and externallyLeads in the development and revision of scientifically based clinical standards for a selected patient populationWorks as a team member to evaluate compliance with professional standardsParticipates with nursing administration in the hiring, supervising and evaluation of nursing staffParticipates and provides leadership on committees and task forces at all levels of the organizationParticipates and provides leadership in the strategic planning process for the clinical program(s)Demonstrates leadership skills, expertise, and in-depth knowledge base consistent with advanced nursing practiceImplements change using relevant theory and data by applying quality improvement principlesUtilizes principles of the consultative process to effectively communicate and contribute to the development of systems related to patient care, organization and health care outcomesMandates internal/external referrals to facilitate best clinical practiceInitiates and collaborates in the strategic planning for program development, implementation and evaluation across the organization for specific patient populationArticulates and demonstrates the integration of the role through research, education, consulting and managing specific patient populationAdvocates for patients and peers by sensitizing interdisciplinary members to issues/dilemmas that require attention and consideration toward positive resultsIdentifies research questions and conducts research to determine best practiceProvides and critically analyses data relevant to system/program decision-makingParticipates in local, regional, provincial, national and international forums to develop policy and standardsInfluences nursing practice by stimulating critical thinkingActs as a research resource to the interdisciplinary teamAnalyzes and interprets current research / literature / knowledge applicable to health care practice with the identified patient populationIdentifies learning needs of peers and provides opportunities for continued education as it relates to clinical practice and patient careDisseminates findings in formats relevant to patients, peers and other colleaguesFacilitates own professional development and maintains clinical competency through formal educational programs and monitoring opportunities with colleagues.Contributes to a transparent culture of client and staff safety by adhering to and abiding by patient and staff safety policies and procedures set by SJHCPromotes an environment that encourages and supports change using change leadership theoryThis position may be required to perform additional related job functions beyond this description. Qualifications & SkillsSuccessful completion of a Master’s degree from an accredited Nurse Practitioner ProgramMember in good standing of the College of Nurses of Ontario as a Registered Nurse in the Extended ClassProfessional Liability InsuranceBCLSWound care training and proficiency (preferred)Three years nursing experience in specialty areaMembership in RNAO or NPAOMembership in professional organizations appropriate for specialtyDemonstrated leadership and team work skillsExcellent communication skills (verbal and written) and team building skillsWhat We Offer:Enrolment in Healthcare of Ontario Pension Plan (HOOPP)100% Employer-Paid BenefitsStarting at 3 Weeks Vacation/YearFree ParkingEmployee Assistance Program (EAP)St. Joseph's Lifecare Centre appreciates all responses.
