We offer flexibility and supportive programs across regional teams. Pay range: Rate: $25.75-39.76/visit (placement on salary range as per Collective Agreement). Estimated annual earning potential: $80,000–$130,000. Actual earnings may vary based on availability and service volumes. Grounded in our commitment to Client and Family Centred Care, the nursing role focuses on providing and coordinating home health nursing services to diverse individuals, families and groups according to the College of Nurses of Ontario Practice Standards, VHA policies, program requirements, and community health nursing standards. Responsibilities Assess, monitor, and coordinate care planning. Evaluate care plan interventions through ongoing reassessment and results. Ensure quality of care by identifying factors affecting practice autonomy and delivery of care. Demonstrate professionalism, leadership, judgment, and accountability in interactions with clients and families. Support safe client care through development of knowledge and skills in line with continuous quality improvement. Inform nursing supervisor of client activities and changes in the care plan; submit appropriate documentation to the case coordinator. Maintain documentation standards in accordance with the College of Nurses of Ontario. Qualifications Registered and in good standing with the College of Nurses of Ontario (CNO). Membership of your Professional Association preferred (RNAO/WeRPN). RN diploma/degree or RPN diploma from a recognized university or college. Valid First Aid or CPR Certification. Wound care or venipuncture knowledge/experience is an asset. Acute Care and Medical-Surgical nursing experience is an asset. Knowledge of community nursing is an asset. Effective written and verbal communication and documentation skills. Reliable attendance, punctuality, and availability for the role. Commitment to lifelong learning and professional growth; ability to apply critical thinking to client care. Valid Ontario driver’s license (G) and a reliable vehicle for traveling.
