Working collaboratively across the Substance Use & Concurrent Disorders (SUCD) Program and the Youth Psychiatry Outpatient Program, the NP provides person-centred, trauma-informed, and recovery-oriented care to individuals with complex mental health and addiction needs. Care is delivered in hospital, community, residential, withdrawal management, and public health settings, including outreach and supervised consumption sites. The NP practices both independently and collaboratively within an interprofessional team, integrating evidence-informed care and harm reduction principles to promote health, manage illness, and support recovery and wellness across the lifespan. Duties Perform comprehensive physical and mental health assessments, including detailed health histories, medication reviews, and system-based evaluations. Integrate clinical findings using evidence-based practice to formulate diagnoses and develop individualized care plans in collaboration with General Practitioners, Psychiatrists, and interdisciplinary team members. Monitors ongoing physical health conditions for patients with stable and chronic illnesses. Monitors medical complications and conditions related to psychopharmacological drug use. Collaborates with patients and their interdisciplinary team members to identify pharmacological and non-pharmacological interventions to achieve best possible health. Manage acute and chronic medical conditions, including those associated with psychopharmacological treatments. Leads and participates in initiatives driving health and mental health promotion. Provides metabolic clinical oversight related to psychotropic medications. Conduct routine health reviews and ongoing monitoring of metabolic and physical health disorders. Consultative support services: email and/or telephone supportive care to staff, extended health care staff and local hospitals, health units for inpatients of the ROHCG. Medical officer responsibilities for outbreaks on inpatient units. Identify and manage co-occurring physical health issues such as Hepatitis C, HIV, and overdose risk. Utilize motivational interviewing and trauma-informed approaches to support patient readiness and recovery. Work collaboratively with primary care providers, hospital teams, withdrawal management programs, and community agencies to build capacity for managing substance use and mental health disorders. Offer mentorship and educational support to students, clinical trainees, and healthcare professionals. Promote evidence-informed practice and contribute to research and knowledge translation activities. Advocates for supportive and safe client care as well as collaborative practice and utilization of evidence-informed practices, in conjunction with other primary care providers at The Royal including the Nurse Practitioner at other sites. Qualifications Master’s Degree in Nursing or BSCN with specialty certification as Nurse Practitioner is required Minimum 5 years of experience is required Current registration as a Registered Nurse in the Province of Ontario in the Extended Class is required (CNO) Specialty certification as a Nurse Practitioner – Primary Health Care (NP-PHC) or Nurse Practitioner – Adult Master’s preparation as a Nurse Practitioner or BSCN with specialty certification as a Nurse Practitioner CPR Able to meet the Standards of nursing practice of the College of Nurses of Ontario. Able to demonstrate achievement of the nurse practitioner competencies of professional role, responsibility and accountability, health assessment and diagnosis, therapeutic management and health promotion and prevention of illness and injury. Able to demonstrate knowledge of relevant legislation and incorporation of this legislation into practice (Mental health Act, Health Care Consent Act, Substitute Decision Act, PHIPA and CSA). Recognized leadership skills with the ability to build the inter-disciplinary team approach to care delivery. Able to work effectively both independently and as a member of a professional inter-disciplinary treatment team. Excellent written and communication skills Valid Ontario’s drivers license (travel may be required) English level A- is mandatory in oral comprehension, expression, reading, and writing.
