Salary: $68.04 – 73.29/hour

About Us
Harbourview Family Health Team (HFHT) is a well-established primary care team based in Thunder Bay, dedicated to delivering high-quality, team-based care to our patients and their families. Our interdisciplinary team includes nurse practitioners (NP), registered nurses, registered dietitians, social workers, administrative professionals, and 14 family physicians.
HFHT is currently in an exciting phase of growth and transition. As we expand and evolve to better meet the needs of our community, we are focused on strengthening our collaborative care model, enhancing patient access, and supporting innovation in primary care delivery.

Job Summary
We are seeking qualified applicants for our core care teams which provide an integrated, team-based approach to care with a team of physicians, nurse practitioner, social worker and registered practical nurses. These teams are designed to enable patients to receive the right care at the right time. The core teams work with a specific patient panel, providing better access to and continuity of care.
The NP is an integrated member of a core team, working to their full scope of practice to support the management of the pod’s collective patient panel. The NP focuses on continuity, proactive management, and panel support. This includes episodic care, follow-up on investigations and treatment plans, inbox management, and collaborative care planning with the core pod team.
Key Responsibilities
- Practice within the Nurse Practitioner as defined by the College of Nurses of Ontario Nurse Practitioner Standards
- Perform comprehensive health assessments within the scope of the Nurse Practitioner role
- Formulate and clearly communicate medical diagnoses to patient and care team
- Support longitudinal management of chronic and complex conditions in collaboration with the core care team
- Facilitate follow-up from investigations, referrals, and care plans initiated by care team
- Provide EMR inbox coverage for physicians following clinic guidelines
- Work collaboratively with interdisciplinary team members to support patient care planning and coordination
- Collaborate and liaise with patients, families and community stakeholders
- Provide education, counselling, and care navigation for patients and families
- Deliver culturally safe, patient-centred care
- Manage test results, prescription renewals, patient communications, and administrative requests
- Active participation in quality improvement initiatives related to workflow design, efficiency, and effectiveness
- Contribute to a positive team environment
- Maintain professional relationships with service providers and related organizations
- Ensure confidentiality and exercise appropriate discretion when handling sensitive client and personnel information
- Follow all organizational policies and procedures to maintain a safe work environment
- Attend required training sessions and comply with all health and safety standards, including WHMIS
- Contribute to teaching and mentoring of learners, where applicable.
- Engage in ongoing professional development
The position is in-person and works in close partnership with a core team, including physicians, nurse practitioners, registered practical nurses, and social workers. The Nurse Practitioners are an integral part of our service delivery, but are also an asset to our team and organizational culture.
Qualifications
- Current certification of registration with the College of Nurses of Ontario (CNO) — required
- Current licensure as a Nurse Practitioner RN(EC) — required
- Active membership in the Registered Nurses’ Association of Ontario (RNAO) and Nurse Practitioners’ Association of Ontario (NPAO) — required
- Demonstrated experience in a primary care or family health team setting
- Proficient in evidence-based practices for primary care delivery, with a focus on health promotion, disease prevention, and chronic disease management
- Advanced clinical and assessment skills, with demonstrated competence in collaboration, critical thinking, and innovative problem‑solving
- Strong organizational and time‑management abilities
- Effective both independently and as part of an interdisciplinary team
- Proven ability to take initiative and contribute to team activities, organizational development, and quality improvement initiatives
- Demonstrates leadership within the clinical team and a commitment to supporting a collaborative, team-based care model
- Bilingualism (English/French or other languages) is an asset
