Nurse Practitioner – Patient Assessment Transition Home – Locum

Salary The salary range for this position is CAD $59.56 - $85.62 / hour. Job Summary Join our team as a Nurse Practitioner in Patient Assessment and Transition to Home (PATH) and bring your clinical expertise and passion for compassionate care providing Locum coverage to Peace Arch Hospital and the communities of South Surrey and White Rock, B.C. This relief part‑time role offers flexible hours throughout the week and is available until May 2026 or the return of the incumbent. Overview The PATH unit is an inpatient hospital‑based program in Fraser Health. Nurse Practitioners assume the role of the Most Responsible Provider (MRP) for patients; they may participate in compensated on‑call work to keep the unit open 24/7, 365 days a year. NPs in PATH units support and empower medically stable alternative level of care (ALC) designated patients with complex discharge requirements and facilitate successful transitions home to thrive in the community. Key opportunities include complex discharge planning, advanced clinical decision making for patients with multiple chronic conditions, geriatric medical care principles, and teamwork with a specialized discharge team to minimize hospitalizations and improve quality of life. Additional clinical enhancement training will be provided, such as ECG interpretation, point‑of‑care ultrasound (POCUS), suturing, and X‑ray interpretation, alongside a centralized NP orientation program. Responsibilities Diagnose and treat patients for undifferentiated diseases, disorders, and conditions within scope of practice, write orders, and provide first line care in emergencies. Monitor ongoing care and order appropriate screening, diagnostic investigations; interpret results to plan treatment. Establish health priorities, follow‑up treatment, and communicate findings, diagnoses, and prognoses with patients, families, and communities. Collaborate with physicians or other health providers to assess and diagnose patient status; develop and implement treatment plans; admit and discharge patients per policy. Prescribe drugs within statutory and regulatory standards and employer policies. Assign work to nursing and health personnel and provide supervision and education as necessary. Participate in research to improve patient care and contribute to nursing and population health advancement. Implement screening and health promotion for at‑risk populations. Participate in interdisciplinary staff and nursing education through case presentations, mentoring, role modeling, and knowledge exchange. Develop, implement, and evaluate policies and procedures related to nursing and health system practices. Perform other duties as required. Qualifications Education and Experience • Current practicing registration as a Nurse Practitioner with the British Columbia College of Nurses and Midwives (BCCNM). • Recent relevant clinical nursing experience.
