The Resident Care Manager reports to the Director of Care and is responsible for the overall provision of personal support care and services to residents, and to ensure adequate resources are available to provide quality person-centered care for all residents. The Resident Care Manager participates in continuous quality improvement initiatives, and acts as a resource, mentor, and coach to Personal Support Workers in various clinical areas, as outlined in the position summary. Responsibilities Provides direction to the PSW team in aspects of resident care Promotes the philosophy of person-centered care, teamwork and a healthy work environment Promotes the Philosophy of Nursing Restorative Promotes the Butterfly Approach to care and exemplifies meaningful engagement to improve the lived experience of the residents Models, listens and assists with conflict and problem resolution Participates in the review of policies and procedures in the personal support services area Chairs monthly Personal Support Worker meetings Responsible for the orientation of new PSWs (home & agency) to the home’s systems, policies and procedures, PSW medication administration (if applicable), and PCC Monitors and evaluates documentation of PSW staff Supervises Personal Support Workers' performance and resident routines and activities Responsible for understanding and employing the collective agreement In collaboration with the Nursing Administrative Assistant, oversees the schedule of PSWs and assists with the replacement of sick calls if needed In collaboration with the Director of Care and the Employee Engagement Specialist, is responsible for the recruitment and retention needs of Personal Support Workers Counsels and disciplines Personal Support Workers In collaboration with the Director of Care is involved with the termination of Personal Support Workers Assists with the implementation of recognition programs Administers medication in the scope of a Personal Support Worker and mentors and coaches other PSWs in this skill On-going delivery of customer service with staff, residents, families, and the general public Mentorship and Preceptor Program-Meeting with preceptors and mentors – helping to guide and develop the staff who are participating in the program Works with Continence Promotion Leaders to stay within the continence product monthly quota Follows through on areas/issues identified through the QI program Participates in CQI activities and QIP processes Conducts audits as assigned Identifies potential liability issues and takes action to minimize risks Required to understand the nature and meaning of quality indicators used by the Home and to understand which indicators are publicly reported Performs daily rounds on all resident home areas and touches base with staff Monitors and teaches the role of a PSW in all required programs Assist with admission and discharge process (i.e. auditing of rooms) Ensures Nursing Restorative Therapy is being performed and documented Other duties as assigned and or indicated in the Job Task Inventory Qualifications PSW Certification from a program that is at least six hundred (600) hours in duration counting both class time and practical experience The PSW program must meet the program requirements set by the Ministry of Colleges or Universities for an Ontario postsecondary institution or district school board to issue a personal support worker certificate Registered with Health and Supportive Care Providers Oversight Authority (HSCPOA) preferred An RN or RPN who in the opinion of the Director of Care has adequate skills and knowledge to perform the duties of a Personal Support Worker A student who is enrolled in an educational program for RN or RPN and who in the opinion of the Director of Care has adequate skills and knowledge to perform the duties of a Personal Support Worker Certification in Medication Administration for Support Workers Ability to communicate both orally and in writing Good interpersonal skills Managerial skills Good interpersonal, leadership and computer skills Training in P.I.E.C.E.S and/or GPA Training in Nursing Restorative Flexibility with work hours Minimum of 1 year experience in Long Term Care To show evidence of being focused on people’s feelings when interacting and knowing how to turn a task into positive social interaction To undertake and meet all legal requirements within a context of promoting individuals’ rights and measuring/reducing elements of risk Must provide a complete and current (within six months) Vulnerable Sector Check (including a Criminal Background Check) or be willing to obtain one As a condition of employment, candidates are required to submit documented results of TB testing (within the last six months or within 14 days upon hire) or a chest X-ray (within the last year), as per Public Health requirements Two supervisory references required Provide proof of all required vaccines.
