The team works with clients to provide holistic care, support and wrap‑around services, with a focus on social determinants of health and ensuring access for those facing barriers such as culture, gender, geography, homelessness, language, disability, poverty and race. Position Overview The Nurse Practitioner will provide comprehensive, client‑centered primary health care emphasizing accessibility, health promotion, illness prevention and continuity of care for individuals, couples and families who may experience complex physical and mental health needs. The NP will practice to their full scope as set out by the College of Nurses of Ontario (CNO) guidelines. Key Responsibilities – Primary Health Care Provide client‑centered primary care to individuals, couples and families with a non‑judgmental stance, considering social determinants of health. Actively participate and provide consultation within an inter‑professional model of care, collaborating with NPs, physicians, RNs and therapists. Perform excellent clinical assessments and deliver treatment within full scope of practice as defined by CNO. Demonstrate leadership within an inter‑professional, proactive team, communicating effectively with staff and clients. Manage evidence‑based conflict resolution and crisis situations professionally. Hold a Prescribing Narcotics and Controlled Substances certificate endorsed by CNO. Work flexible hours, including evening work. Address biopsychosocial needs of clients, including comorbid health conditions, mental health diagnoses and substance use disorders. Provide counseling, education, health promotion, internal referrals and follow‑up. Consult with external health service providers in the client’s circle of care. Engage in community development, outreach and resource management with partners. Provide case management and follow‑up for treatment regimes. Maintain timely and accurate medical records and documentation using DCHC’s EMR (Telus PSS). Participate in development, implementation and evaluation of medical directives, policies, protocols and best‑practice procedures. Develop health‑promotion services; trans health is an asset. Advocate for at‑risk clients and families. Participate in case conferences as required. Engage in chart reviews, quality‑improvement planning and accreditation activities with the Canadian Centre for Accreditation (CCA). Contribute to the CNO Quality Assurance program and DCHC performance‑management requirements. Participate in designing, implementing and evaluating research studies in partnership with universities or clinical student placements. Attend additional DCHC training activities and meetings. Conduct home visits for clients living in the community. Provide supervision and training for NP students when required. Ensure client caseload aligns with DCHC's Multi‑Sector Service Accountability Agreement (MSAA) indicator targets. Perform other duties as assigned within scope of practice. Work flexibly from any DCHC clinical location and travel to meetings across service area. Work at least one evening per week. Key Qualifications MSN (Master’s in Nursing). Completion of a recognized primary‑care NP program (NP‑PHC). Registered RN (Extended Class) in good standing with CNO. Member of the Nurse Practitioners Association of Ontario (NPAO) or RNAO (asset). Minimum three years’ experience as a primary‑care NP serving clients with complex needs; child and youth experience required. Experience in a CHC or community health nursing setting (asset). Experience working with clients facing barriers to traditional health care. Excellent critical‑thinking, problem‑solving, organizational and interpersonal skills; collaborative team player. Effective non‑judgmental communication with clients, active listening and coaching. Capacity to manage crises professionally. Excellent written communication and report‑writing for clinical settings. Basic CPR certification (required). Proficiency with Microsoft Office (asset). Competency in facilitating client group activities. Valid current vulnerable‑sector police check (required upon hiring). Valid Ontario driver’s licence and access to a vehicle. Demonstrated Minimum Attributes Service‑oriented, results‑oriented, adaptable, collaborative, effective communication skills, team player, risk management and accountability.
