Nurse Practitioner - Black Health and Wellness Durham Community Health Centre (DCHC) is seeking a Nurse Practitioner (NP) to deliver comprehensive, client-centered primary health care with a focus on high-priority populations within the Black Health and Wellness program. Pay Annualized salary range: $121,119.00 – $125,916.00, commensurate with skills and experience. Benefits include Group Benefits & HOOPP. Location 1320 Airport Blvd, Oshawa and mobile clinic (flexibility to transition to other DCHC sites as required). Position Overview As a member of an interdisciplinary team, the NP will provide primary health care assessments, screenings, prevention, treatment, referrals, and activation of clinical pathways for community members within high-priority populations. Key Responsibilities Provide client-centered primary care in a non-judgmental manner. Actively participate in an inter-professional model of care. Perform clinical assessments and health information gathering for clients with complex needs. Deliver nursing services addressing biomedical, psychosocial and mental health needs. Complete intake assessment for new clients. Contribute to targeted services for high-priority populations, including newcomers and ACB communities. Complete accurate medical records using DCHC EMR system. Support client safety through evidence-based practices and adherence to policies. Contribute to development of medical directives, policies and protocols. Adhere to DCHC clinic operational guidelines. Participate in case conferences and chart audits. Implement QIP and accreditation activities with Canadian Centre for Accreditation (CCA). Maintain CNO Quality Assurance and DCHC performance management. Participate in training activities and meetings. Provide clinical supervision and training for nursing students. Ensure caseload management aligned with Multi-Sector Service Accountability Agreement (MSAA). Key Qualifications Master’s Degree in Nursing (MSN). Completion of recognized primary care NP program (NP‑PHC) and certification with the CNO. Three years of primary care NP experience with clients having complex needs; child and youth experience required. Experience with priority populations such as newcomers and ACB communities. Knowledge of Durham Region community supports highly preferred. Valid prescribing narcotics and controlled substances certificate endorsed by CNO. Experience in CHC or community health nursing practice preferred. Experience working with populations facing barriers to traditional health care. Excellent critical thinking, problem-solving, organizational and interpersonal skills. Effective communication skills with clients, demonstrating active listening and collaborative partnership. Ability to manage crises professionally and calmly. Strong written communication and report‑writing ability. Basic CPR certification required. Proficiency with Microsoft Office applications (Word) an asset. Valid current vulnerable sector police check required upon hiring. Valid Ontario driver’s licence with access to a vehicle. Flexibility to work or drive to meetings at any of DCHC’s clinical locations, including mobile clinic sites. Ability to work a flexible schedule with at least one evening per week.
