We are funded by the Ministry of Health, Ontario Health, Ministry of Community, Children and Social Services, the City of Toronto and many generous donors and foundations.about the role:Sherbourne Health and The Neighbourhood Organisation co-lead the Health Access St. James Town initiative, a collaboration of 15 health and social service providers. The Health Access Expansion Project will focus on expanding the existing Health Access St. James Town model to surrounding neighbourhoods.This initiative aims to create and improve access to health care and other community support services by coordinating and aligning with client health care needs, including equitable pandemic response and sustainable healthcare response to residents, specifically those who are facing multiple barriers to health care and social supports, with a focus on black and indigenous populations.The Community Outreach Worker will be mainly responsible for community engagement and outreach initiatives.
