Current Basic Cardiac Life Support (BCLS) Training as delineated in Prairie Mountain Health Policy PPG-00002 CPR Training Province of Manitoba Class 5 Drivers Licence, or equivalent from province of residence, and access to a personal vehicle to provide service within Prairie Mountain Health Proficiency with Microsoft programs (Outlook, Word, Excel, Access and PowerPoint), as well as Internet applications and other Information Technology Demonstrated organizational skills and ability to work independently Demonstrated problem solving and decision making skills Demonstrated flexibility to facilitate changes in techniques and procedures in a changing environment Demonstrated knowledge and competence of skills and concepts related to the position Demonstrated communication skills Ability to respect and promote confidentiality Ability to perform the duties of the position on a regular basis Ability to respect and promote a culturally diverse population Ability to work effectively and maintain positive working relationships with co‑workers, clients and within interdisciplinary team Position Summary The Community Nurse is an integral member of the interdisciplinary team. The nursing role encompasses a high degree of independence and autonomy as well as interprofessional collaborative practice. The Community Nurse provides service to individuals and groups in their natural settings, as well as in clinic, with a focus on increasing primary care access for marginalized populations. The Community Nurse operates using a harm reduction lens and philosophy, including offering harm reduction supplies, harm reduction education, and sexually transmitted blood borne infection (STBBI) testing (in collaboration with the primary care provider). The Nurse III recognizes and fulfills their professional responsibilities by practicing in a manner consistent with professional obligations to take actions as per the Canadian Nurses Association Code of Ethics for Registered Nurses, The College of Registered Nurses of Manitoba Standards of Practice for Registered Nurses, the scope of practice as defined by the Regulated Health Professions Act (RHPA), and other Provincial and Federal legislation. Responsibilities Practices as a member of a collaborative interdisciplinary team. Provides primary care interventions including client‑centered assessments of health status. Consults with prescribers/associated care providers, administers prescribed medications, and treats minor conditions within the scope of nursing practice. Uses a harm reduction approach, meeting the client(s) where they are at, and empowering client(s) to access health and social services, while supporting behavior change. Identifies the health issues, behaviors, needs, barriers and challenges often faced by clients. Works collaboratively to develop creative strategies to address challenges and barriers identified. Develops a mutually agreed upon plan and priorities for care with the individual and family and/or substitute decision maker. Supports clients to access and attend medical appointments, hospitals, housing services and social programs by providing health care information, facilitating referrals, and/or arranging transportation. Engages in community outreach to deliver services for individuals and groups in their natural settings, and to mobilize and empower clients to access local health and social services. Outreach occurs in non‑traditional settings such as community spaces, agencies, drop‑ins, shelters, encampments, residences, withdrawal management centers, hotels, parks, and streets. Uses knowledge of the community to link with, refer to, or develop appropriate community resources. Applies the nursing process by assessing the needs of clients, families, and groups considering the determinants of health, age and developmental stage, physical and psycho‑social health status; analyzing, developing and implementing a plan of care, and evaluating the outcomes based on professional judgement and skills to guide decision‑making for the delivery of client care/services through various methods. Conducts screenings and risk assessments as per clinical practice guidelines, including for STBBIs. Provides care that is culturally competent and respectful of diversity (eg. economic and social status, race, culture, belief systems, sexuality). Identifies and addresses client learning needs and readiness. Employs health promotion, health education and harm reduction strategies to support behavioral changes conducive to health (e.g., smoking cessation, physical activity, healthy eating and mental wellbeing) jointly with the client, defining and incorporating these strategies into the health plan. Supports the client in self‑management of chronic disease. Responds appropriately to crisis situations. Completes documentation and administrative requirements as required. Monitors and orders necessary supplies and medications. Ensures clinical equipment is in working order and addresses any malfunctions or deficits. Participates in the orientation and training of new staff. Provides supervision, mentorship, orientation and teaching opportunities to students, ensuring that program goals and objectives are met. Informs manager of any unsafe acts, working conditions, incidents, near misses, injuries or illnesses, workload issues, issues of a political nature or media interest. Other duties as assigned.
