The Community Wellness Coordinator delivers a variety of client participant-based services to individuals and families with the primary focus to address their health, healing, and wellness concerns. The CWW Coordinators assist individuals to identify concerns and develop action plans to address goals, barriers and resolution. Coordinators also deliver educational workshops to improve the wellbeing of the community as a whole. They offer programming for people of all ages, to ensure culturally vibrant and safe way of life is achievable for everyone, in line with MNO’s Statement of Prime Purpose. What you will be doing: Ensure that Community Wellness Worker program is delivered in the spirit and philosophy of the Métis Nation of Ontario’s Statement of Prime Purpose Facilitate client services such as home visiting, service support assessments, referrals, developing a wellness plan, and attend consultations with client. Coordinate traditional based activities which involve the specific use or development of traditional Métis knowledge, practices, teachings, or services to achieve the objectives of the activity or event. Provide education through organized, facilitated, or coordinated activities. Participant based activities are group rather than individual activities that are open to a targeted group of people within the community or the community at large. Maintain client data in the Online Health Activity Tracking System (OHATS) and program documentation. Maintain confidentiality at all times, and adhere to privacy legislation Perform Administrative duties, and other duties as instructed by the Program Supervisor Spend local budgets according to approved plan and complete financial reporting Reflect Métis cultural approaches as a part of the activities and services Promote the values of the organization within the workplace and in the community by demonstrating positive and professional relationships with others (employees, citizens, council, volunteers, advocates, etc.) Report all unsafe conditions, hazards or practices, accidents and incidents in accordance with the Occupational Health and Safety Act and adheres to all Health and Safety requirements within the MNO’s policies and procedures and provincial legislation. Perform other job/branch-related duties as required. Qualifications: 1 to 3 years of experience in client case management and hosting workshops or activities for community College Diploma (1 or 2 year) in social services Experience integrating trauma-informed approaches into their work Strong desire to act in the service of the Métis Citizens, to advance and uphold rights and culture Knowledge of the Métis Nation of Ontario and programs offered Knowledge of Indigenous programs and services Knowledge of health and wellbeing topics Knowledge of trauma-informed approaches Ability to facilitate workshops Ability to work as part of a team and independently with minimal supervision Well-developed administrative, organizational and communication skills in order to meet deadlines Ability to lift 11.5kg when required Proficiency with computer programs (Outlook, Excel, Word and PowerPoint) Valid G-class license Access to reliable vehicle Willing and able to travel for the purpose of meeting program objectives and attend training. Willing and able to occasionally work hours outside of the regular Monday‑Friday, 8:30am – 4:30pm schedule.
