Home and Community Care Coordinator
Full-Time (37.5 hours per week) – Temporary 1 year Maternity Leave Coverage
Salary Range:  $57,795 - $69,010 Annually
Location:  4 First Nation communities; Garden River First Nation, Batchewana First Nation and the Urban Indigenous population through the IFC Health Clinic

Maamwesying North Shore Community Health Services provides a full and coordinated continuum of care within the seven-member First Nation communities of the Mamaweswen, North Shore Tribal Council and to the urban Aboriginals residing in the city of Sault Ste. Marie through the Indigenous Friendship Centre.  Services include N'Mninoeyaa Aboriginal Health Access (one of ten AHACs in Ontario), Northern Clinic, Mental Health and Addictions Services, Chronic Disease Prevention, and Traditional Health and Healing, Home and Community Support Services.  We are currently seeking a compassionate and organized individual to fill the temporary role of Home Care Coordinator during a maternity leave.

Key Responsibilities:

  • Assessment and Eligibility: Determine client needs and eligibility for services based on program guidelines.
  • Care Plan Development: Establish and implement multi-disciplinary care plans
    Service Coordination: Coordinate services with community agencies, hospitals, and other relevant parties.
  • Case Management: Collaborate with a multi-disciplinary team in the shared care service delivery model of care.
  • Resource Provision: Assist clients in accessing placement services, medical supplies, and other necessary resources.
  • Client Support: Provide ongoing support and advocacy for clients, ensuring their needs are met.
  • Communication: Maintain effective communication with clients, families, and other professionals involved in care.
  • Documentation: Maintain accurate and up-to-date records of client information and care plans.
  • Advocacy: Advocate for clients' rights and needs within the community.
  • Program Support: Support and assist in various health center programs.
  • Strong Communication Skills: Effective communication with clients, families, and other healthcare professionals is crucial.
  • Assessment and Planning Skills: The ability to assess client needs and develop comprehensive care plans is essential.
  • Organizational Skills: The ability to manage multiple tasks and coordinate services effectively is necessary.
  • Problem-Solving Skills: The ability to identify and address challenges that may arise in client care.
  • Empathy and Compassion: The ability to understand and connect with clients' needs and experiences.
  • Knowledge of Community Resources: Familiarity with First Nation's level health care services, local resources and services available to clients.

Qualifications:

  • Post-secondary education in Health Sciences, Social Work, Indigenous Studies, Gerontology, or a related field.
  • Experience in care coordination, case management, or community support services.
  • Strong knowledge of Indigenous cultures, traditions, and health perspectives.
  • Excellent organizational, communication, and problem-solving skills.
  • Ability to work independently and as part of a collaborative team.
  • Valid driver's license and access to a reliable vehicle for travel within the region.

Assets:

  • Experience working in or with Indigenous communities.
  • Understanding of the Non-Insured Health Benefits (NIHB) Program and other health-related supports.
  • Knowledge of local resources and service providers in First Nation and area.
  • Ability to speak an Indigenous language is an asset.
  • Knowledge of interest in the health issues affecting Anishnawbe people. Cultural awareness and sensitivity training an asset.
  • Must have access to a reliable vehicle and a valid G Class driver's license
