Overview We collaborate with community programs such as Community Mental Health, Employment and Income Assistance, Home Care, and our Community Facilitators to connect patients to additional resources. Our partnership with Grace Hospital provides access to laboratory and diagnostic services. The team includes Physicians, Nurse Practitioners, Primary Care Nurses, Primary Care Assistants, Allied Health professionals (psychology, occupational therapy, physiotherapy, dietitian, pharmacy, midwifery), and learners from various health disciplines.
