Must be available to work varied shifts (days, afternoons and evening shifts) including Saturdays, Sundays and holidays Job ID: 56743 Job Category: Health Services Division & Section: Public Health, Strategy & Clinical Services Work Location: Union Station, 61 Front St W, Toronto ON, M5J 1E5 Job Type & Duration: Full-time, 1 Temporary (approximately 24 month) Vacancy Hourly Rate and Wage Grade: $46.34 - $5 0.68, Wage Grade 13 Shift Information: 35 hours per week, Monday to Sunday (includes evening shifts and holidays) Affiliation: L79 Full-time Number of Positions Open: 1 Posting Period: 05-Dec-2025 to 19-Dec-2025 Major Responsibilities Provides public health nursing services for individuals and groups living with mental health and addictions in accordance with TPH program specific policies, procedures and medical directives, as well as the Standards of Practice of the College of Nurses of Ontario and the Community Health Standards of Practice. Provides direct care to clients in clinical settings. Provides clinical services under medical directives in certain situations (e.g., immunizations, physical examinations including but not limited to pelvic examination, Pap smear, and sexually transmitted infection (STIs) testing, treatment based on positive lab result for specific infections, manages anaphylaxis and provides immediate care/action in emergency situations. Performs venipuncture, orders laboratory tests, provides prescription for and/or dispenses medications as per clinic policies and procedures e.g. STD medications, emergency contraceptive pill, oral contraceptives). Reviews laboratory results and follows up as per clinic policies and procedures. Participates in the development of policy, procedures and protocols and updates resources for managerial approval. Maintains individual/client records and necessary documentation according to policies and procedures of TPH as well as the Standards of Practice set out by the College of Nurses Educates clients regarding harm reduction, overdose prevention, recognition and response, naloxone administration, safer drug use, STIs and Blood-bourne Infections (Hepatitis C, Human Immunodeficiency Virus [HIV]/Auto Immune Deficiency Syndrome [AIDs] and Syphillis) prevention, sexual abuse/assault, life skills, sexuality, contraceptives, abortion, and pregnancy. Performs needle exchange, distributes safer drug use kits and provides education on use of safe sex materials, safer drug use from office location, van, clinics, on the street, in community centers and/or shelters Provide direct client care in work various clinical offices, outreach and community-based settings including assess mental and physical health status of clients, prepare and administer controlled substance such as Methadone, Suboxone, monitor clients post dose administration and completing follow-up interventions. Provides various physical and diagnostic interventions including wound care, electrocardiogram (ECGs) as ordered by prescribers. Collaborates and liaises with other professionals, community agency staff and City of Toronto staff to identify health needs and participates in the development of community partnerships and action plans to meet those needs. Composes and signs correspondence in relation to clients' needs and advocates on clients' behalf when liaising with doctors, pharmacists, prison authorities, laboratories, etc. Attends court with clients to act on their behalf. Provides immediate care in crisis situations and performs first aid treatment as necessary. Provides counselling and medical care to clients receiving opioid substitution medication such as Methadone, Suboxone and Sublocade. Works with clients to develop and promote improved health, coping and social skills Accompanies clients requiring emergency medical and/or psychiatric assessment to medical and detox centers Attends meetings as required nursing related and program specific meetings as required. Collects and maintains statistical information for program monitoring and evaluation purposes. Participates in professional conferences and workshops Develops, implements and facilitates specialized groups, workshops and in-services on a variety of health-related topic Participates in community outreach and marketing health promotion strategies Participates in the divisional planning process including development of work plans/operational program plans Participates in the planning, development, implementation and evaluation of public health programs and initiatives which may include community mobilization, advocacy for healthy public policy, health education and skills building Collaborates and liaises with members of the team and community agencies to identify health needs and participates in the development of plans to meet those needs Key Qualifications Your application must describe your qualifications as they relate to: A Baccalaureate degree in nursing from a recognized university and a valid license from the College of Nurses of Ontario. Experience in the provision of harm reduction, drug treatment/addiction counselling, methadone, social, health, housing and HIV/AIDS related services. Experience working in an outreach capacity with people who use drugs, people experiencing mental health issues and/or people who are homeless or underhoused. Must possess a valid Ontario Driver's License Class "G" with the ability to qualify for a City Equipment Operator's License, and able and willing to drive within the Greater Toronto area. You must also have: Ability to conduct work in a non-judgmental, empathetic way, using a harm reduction approach. Excellent assessment skills. Thorough knowledge of reportable communicable diseases such as Hepatitis C, HIV, Syphilis, and MPOX including transmission, prevention and treatment options. Able to foster therapeutic rapport with client while maintaining professional boundaries with clients accessing service. Thorough knowledge of drug use including safer drug use practices, safer supply, opioid substitution Committed to continuous learning, professional development and growth with Nursing. Knowledge of drug treatment, social, housing, health, HIV/AIDS related programs and services. Awareness of issues relating to sex works, addictions, mental health and homelessness. Ability to work flexible hours, which may include overtime and will include some evening and weekend shifts. Proven ability to work as a member of a multidisciplinary team.
