What You Will Do Advanced Clinical Assessments: Provide triage and clinical assessments at the mental health Single Point of Access, ensuring smooth and efficient entry into crisis services. Interventions & Preventive Care: Implement brief interventions to avoid unnecessary hospital admissions or visits to A&E, promoting early intervention and support. Collaboration with Agencies: Build strong relationships with key partners such as police, ambulance services, and local agencies to enhance support networks and improve service delivery. Mental Health Awareness: Contribute to training and mental health awareness initiatives, particularly for acute hospital staff. Care Plan Development: Work closely with service users and their networks to create safe, compassionate, and evidence-based care plans.
