GH-CCMH is also a member of the Thriving Minds Mental Health Access Collaborative with CAMH and SickKids and a core service provider in the Toronto sector of infant, child and youth mental health services funded by the Ministry of Health (MOH). GH-CCMH provides prevention, early intervention and health promotion services across the city, funded by MOH and other sources. As such, the Centre has an ambitious agenda to provide the highest quality, evidence-based care, offering a range of services from prevention and early intervention to counseling and therapy, assessment and intensive supports. Services are offered in person and in virtual format. Find out more about GH-CCMH.We are seeking applicants for a Child and Youth Care Practitioner, Relief position at the Intensive Services for Children and Intensive Services for Youth– Day Treatment program. This role will be responsible for providing direct mental health treatment services to children ages 6-12 and youth ages 13-18 with social, emotional and behavioral problems.KEY RESPONSIBILITIESClinical ServicesMember of an interprofessional teamProvides clinical interventions and services for assigned children, youth and their families maintaining caseloads and supporting clientsParticipates in the development and implementation of individual treatment plans for clients, assessing behaviours and responding to clients’ needsProvides support in the classroom and leads CYCP-led skills based groupsCoordinates cases with family/caregiver/relevant service providers/service planning teamCreates and implements materials for clients/caregivers relating to strategies/interventionsAttends meetings with staff and families who may be accompanied by other staff or supervisorsSupports clients by facilitating social skills/therapeutic activities and organizing/attending eventsPrepares documents/reports related to treatments plans and related activities, as well as consults with others, as neededMaintains up-to-date knowledge of best practices related to clinical servicesMakes decisions within related to day-to-day treatment issues and on-going intervention techniquesUnderstands when unusual or unique situations need to be discussed with supervisor/manager and or interprofessional teamOtherSupervision of CYC students, if requiredComplies with the organization’s vision, policies and proceduresPerforms other tasks as assigned by managementQualificationsPost-secondary Diploma in Child and Youth Care and a minimum of three years related experience or equivalent combination of education and experience; experience working in community is strongly preferredDemonstrated ability to work with at-risk children, youth & families, community partners and organizationsUnderstanding and analysis of issues of marginalization and oppression are necessary to work in this roleMember with the Ontario Child and Youth Care Association or equivalent association/ accrediting organizationStrong verbal and written communication skills to deal with clients and familiesStrong problem-solving skills to analyze client issues and recommend solutions and manage crisis situationsFirst Aid/CPR, and TCI requiredGood knowledge of MS Office Suites (Outlook, Word, Excel, PowerPoint), as well as ability to use office equipment efficiently (telephone, scanner, photocopier and fax)Ability to engage in physical activities related to sports, board games, play based etc., and TCI trainingAbility to travel to both SickKids CCMH LocationsInterested applicants are asked to forward their cover letters and resumes by December 29, 2025, to:Human Resources – SickKids CCMHE-mail: hrrecruiting@sickkidscmh.caIn the subject line of email, please quote the: competition number, position title and where you learned about this opportunity.We are seeking applicants internally and externally.CCMH at SickKids is an equal opportunity employer; we are committed to diversity and value inclusion.
