SickKids Centre for Community Mental Health (SickKids CCMH) is a community-based children’s mental health centre providing prevention, in-home support, day treatment, outpatient assessment and treatment and residential support to children ages birth to 18. SickKids CCMH is integrated with the Hospital for Sick Children (HSC) inpatient and ambulatory mental health services for children. As a stand-alone community-focused agency SickKids CCMH is working with the Hospital for Sick Children to create pathways to care that will ensure that children with mental health concerns and mental illness and their families are able to access the care they need in a timely fashion. SickKids CCMH is an accredited teaching facility affiliated with the University of Toronto.

**We are seeking applicants for a Child and Youth Care Practitioner position who have lived experience and/or familiarity with equity seeking groups and communities. The successful incumbents will be responsible for providing direct mental health treatment services to clients within the Intensive Services for Youth program at**
**SickKids Centre for Community Mental Health. The Intensive Services for Youth program offers a continuum of service that includes day-based programming and community support. This role will primarily be based in the community and will require travel across the catchment area, flexible hours and work in community settings including clients’ homes.**

**Key Responsibilities**:

- Provides clinical interventions and services for assigned children, youth and their families maintaining caseloads and supporting clients
- Participates in the development and implementation of individual treatment plans for clients, assessing behaviours and responding to clients’ needs
- Provides support in the home, in community settings and transitional supports
- Coordinates cases with family/caregiver/relevant service providers/service planning team or creates materials for clients relating to strategies/interventions
- Attends quarterly conferences with staff and families who may be accompanied by other staff or supervisors
- Supports clients by facilitating social skills/therapeutic activities and organizing/attending events
- Prepares documents/reports related to treatments plans and related activities, as well as consults with others, as needed
- Maintains up-to-date knowledge of best practices related to clinical services
- Makes decisions within related to day-to-day treatment issues and on-going intervention techniques
- Understands when unusual or unique situations need to be discussed with supervisor/manager

**Other**:

- Supervision of CYC students, if required
- Complies with the organization’s vision, policies and procedures
- Performs other tasks as assigned by management

**Qualifications**:

- Post-secondary Diploma in Child and Youth Care and a minimum of three years related experience or equivalent combination of education and experience; experience working in community is strongly preferred
- Demonstrated ability to work with at-risk youth & families, community partners and youth organizations
- Understanding and analysis of issues of marginalization and oppression are necessary to work in this role
- Member with the Ontario Child and Youth Care Association or equivalent association/ accrediting organization
- Good verbal and written communication skills to deal with clients and families
- Good problem-solving skills to analyze client issues and recommend solutions and manage crisis situations
- Good knowledge of MS Office Suites (Outlook, Word, Excel, PowerPoint), as well as ability to use office equipment efficiently (telephone, scanner, photocopier and fax)
- Ability to work flexible hours including evenings
- Ability to travel to various locations, including clients’ homes
- Valid driver’s license and access to a vehicle
