Family Physician - Health and Wellness Focused Practice
Job Description:
We are seeking a qualified Family Physician who is passionate about pioneering a patient-centric approach to health care. Our practice emphasizes a proactive model focused on preventive care, long-term health outcomes, and comprehensive patient advocacy. The ideal candidate will be dedicated to delivering personalized medical care, emphasizing early intervention and ongoing health management to enhance quality of life.
Responsibilities:
- Provide continuous and comprehensive care to patients, addressing both immediate and long-term health concerns.
- Develop individualized care plans based on thorough health assessments and patient histories.
- Focus on preventive measures, health education, and lifestyle modifications to prevent disease and promote health.
- Serve as a dedicated advocate for patients, navigating complex health systems to coordinate care effectively.
- Engage with patients to form a trusting, open relationship that fosters communication and facilitates better health outcomes.
Qualifications:
- MD or DO degree from an accredited medical school.
- Board certification in Family Medicine.
- Strong commitment to health prevention, patient education, and long-term wellness.
- Excellent communication skills and the ability to build relationships with patients.
- Experience in a similar health-focused or preventive care role is preferred.
