This is a fully in office role in our Mississauga office. In this role will use all your nursing skills and training to provide remote assessments and triage for patients giving them the confidence they need to remain in their homes for care, where we know they want to be. This hybrid role encompasses in-office and remote work.Job DescriptionThis is a unique opportunity to be part of a tight-knit, collaborative team from across the health sector, you’ll work as part of an integrated and highly coordinated team that provides holistic care to patients, you’ll also provide 24/7 clinical virtual support on a scheduled rotation. Drawing on your expertise and experience, you will provide remote assessment and virtually triage support for home care clients. As the first point of clinical contact for patients, you will assess their needs and determine the appropriate next steps. When in-person care is required, you will coordinate the necessary follow-up support for direct clinical care. You will also support the client and care partners with symptom management, navigation and education.QualificationsThis is a fully in office role (Mississauga office)Registered Nurse (RN) or Registered Practical Nurse (RPN) in good standing with the College of Nurses of OntarioAbility to work in our Mississauga officeMinimum 3 years of clinical experience in a community, medical surgical departments, emergency department, or triage setting preferredDemonstrated ability to perform comprehensive clinical assessments virtually and make independent decisionsAdditional InformationThis position is a current vacancySpectrum Health Care is thrilled to be named one of Canada’s Best Managed Companies.
